Job overviewWe are looking for an individual who is highly motivated and enthusiastic about positively impacting the discharge experience for patients, families, carers and staff, whilst ensuring that people leave the acute setting in an appropriate and timely manner.
This exciting role gives a unique opportunity to work with expert clinical and operational teams within North Bristol Trust, as well as within our Transfer of Care Hub and at wider System level, to ensure we deliver effective, safe and excellent care to our patients.
The post holder will have recent expert knowledge of leading large teams, in either acute or community settings and have a proven track record of delivering meaningful and sustained change.
The successful candidate will possess a strong ability to build successful teams whilst supporting, influencing and motivating staff to deliver challenging targets through times of change.
The post holder is expected to provide proactive and visible leadership across the discharge services and we are looking for someone who is ambitious, focused and passionate about patient care, shaping the challenge and opportunities within the current NHS.

Main duties of the jobBe responsible for the development and delivery of the IDS service, along with support for the provision of the Transfer of Care Hub (ToC Hub).Be responsible for service development, evaluation, research and audit of Discharge Services in NBT.Provide Leadership and consultancy for discharge related inquiries both within NBT and as a lead within the Bristol, North Somerset and South Gloucestershire (BNSSG) Integrated Care System (ICS).Deputise for the Associate Director of Integrated Discharge in appropriate forums and support strategic development of Discharge Strategy across the ICS.This role will require participation in the on-call rota which includes weekends and bank holidays as required.
