Job summary We are seeking to appoint a GPs withan interest in Frailty to support the delivery of the various developingFrailty Services, including but not limited to:The York Frailty CrisisResponse HubYork FrailtyOutreach ServiceThe YorkIntegrated Community Team MDTsLow IntensityVirtual Frailty Ward patients follow upYouwill need to hold the relevant professional registration (GMC) and haveexperience ofTriage / Telephone Assessment.
Experience of working in GeriatricMedicine is desirable.
Any successful candidates would join our growing team ofGPwERs in Frailty and be supported to obtain their Diploma in Geriatric medicineas well as join our regular education sessions.
This is predominantly a face-to-facerole based at Acomb Garth Community Centre between the hours of and 5pmbut if doing any evening shifts between 5 and 8pm then this can be doneremotely from home (evenings paid at enhanced rates).

Main duties of the job You will undertake a variety of duties, offering support and advice to System providers regarding vulnerable patients in Community as part of an MDT, undertake best interest discussions with patients and family members and help co-ordinate a response to support people in need to stay home whenever possible.
More rarely you may be needed to visit a complex frail patient at home or undertake telephone consultations.
Assessing the health care needs of complex patients with undifferentiated and undiagnosed problems, highlighting which services would support them best to stay in their home setting and independent.
Cover the clinical management of patients as appropriate including initiating investigations, reviewing results, making referrals to other providers/services as appropriate.
Compile and issue computer-generated acute and repeat prescriptions.
Prescribe generically whenever appropriately and in accordance with NICE guidance and Good Prescribing Practice (as defined by British National Formulary guidance).
Work with and support other members of the MDT, including but not limited to: social workers, therapists, social prescribers, voluntary support workers, frailty nurses, dieticians, UCR paramedics and other Frailty GPs.
Participate in frailty training and maintain high volume of CPD in Frailty related conditions with the aim of obtaining the Diploma in Geriatric Medicine.

Promote the application of standards and codes of Practice set by the Royal College of General Practice, General Medical Council, Department of Health, British Geriatrics Society and local NHS trust.
Participate in national frailty training and maintain high volume of CPD in Frailty related conditions with the aim of obtaining the Diploma in Geriatric Medicine.Other Responsibilities: Involve and contribute to the mentoring, supervision and training of the broad multi-professional clinical team.
Be aware of and comply with all relevant policies/guidelines, prescribing, confidentiality, data protection, health and safety.
