Job summaryThis is a position for a newly qualified, enthusiastic, andforward-thinking GP to join Swanley & Rural PCN in Kent & Medway.Currently the role is fixed term up until 31st March 2025.As this post is funded through the Network Contract DES, youmust be within 2 years of GP qualification and not have been substantivelyemployed as a GP previously.This post will require you to work across two practiceswithin the PCN all of which are rated as Good with CQC.
This presents a greatopportunity to gain valuable experience and insight of the workings and culturewithin different practices.
Main duties of the job Main duties of the jobYou will manage a case load and will work autonomously on aday-to-day basis.

Job description Job responsibilities Job responsibilitiesClinical responsibilities:The post-holder will be required to undertake a variety ofduties including surgery consultations, telephone consultations and queries,checking and signing/authorising repeat prescriptions and dealing with queries,paperwork and correspondence in a timely fashion.Making professional, autonomous decisions in relation topresenting problems, whether self-referred or referred from other health careworkers within the organisation.Assessing the health care needs of patients withundifferentiated and undiagnosed problems.Screening patients for disease risk factors and early signsof illness in consultation with patients and in line with current practicedisease management protocols, developing care plans for health.Admitting or discharging patients to and from the caseloadand referring to other care providers as appropriate.Recording clear and concise consultation notesCollecting data for audit purposes.Prescribing in accordance with local formulary and nationalguidelinesThe post-holder may be required to visit patients duringsurgery time in an emergency if clinically indicated.The post-holder may be required to support enhanced accesssessions as per the needs of the PCN.CommunicationUtilise and demonstrate sensitive communication styles, toensure patients are fully informed and consent to treatment.Recognise the need for alternative methods of communicationto overcome different levels of understanding, cultural background andpreferred ways of communicating.Delivering a quality servicePrioritise, organise and manage own workload in a mannerthat maintains and promotes quality, including participating in shared learningacross the practice, PCN and wider organisation.
Work as part of the team toseek feedback, continually improve the service and contribute to businessplanning.Learning and developmentTake responsibility for own learning and performance throughcontinuous professional development, including participating in supervision,acting as a positive role model and assessing own learning needs.Act as a mentor and positive role model to students andjunior members of staff, sharing information and good practice.Contribute to education and training of trainee clinicalstaff and students.Maintaining confidentialityThe GP will, as an integral part of and in the course ofcarrying out his/her role, have access to confidential and sometimes sensitiveinformation relating to patients, carers and family members, as well as similarinformation about colleagues.
They may also have access to confidentialinformation about the practices and the PCN.All such information, in whatever format and howsoever madeavailable, must be treated with the strictest confidence.

Person Specification Knowledge and Skills Essential Essential Ability to work within own scope of practice and understanding when to refer to GPs Ability to work autonomously, and as part of a multi-disciplinary team Ability to evaluate the safety and effectiveness of own clinical practice Understanding of evidence-based practice Ability to effectively triage, assess and diagnose Ability to request and review pathology tests and other relevant investigations and advise patients accordingly Assessing and managing acute deteriorations/exacerbations of LTCs Understanding of evidence-based practice Ability to assess and manage patient risk effectively and safely.
Broad knowledge of clinical governance Excellent communication skills (written and oral) including recording of accurate clinical notes Strong IT skills incl.
competence in the use of Microsoft Office and Outlook Effective time management Ability to listen and empathise whilst being honest and caring Polite and confident, flexible and cooperative Motivated, forward thinker with ability to use own judgement and common sense Problem solver with the ability to process information accurately and effectively, interpreting data as required Ability to work under pressure Effectively able to communicate and understand the needs of the patient Commitment to ongoing professional development Good time management Observance of strict confidentiality Disclosure Barring Service (DBS) check Full UK driving licence with access to own vehicle Experience Essential Essential Experience of working in primary care Experience using a primary care IT system such as EMIS or System One Experience of QOF and clinical audit Experience of dealing with a range of clinical conditions General understanding of GMS contract Desirable Experience using EMIS Qualifications Essential Qualifications Essential Newly qualified GP, within 2 years and not having had a substantive post.
Completed MRCGP Full GMC registration and license to practice Eligibility to practice in the UK independently Ensure Continuous Professional Development is maintained
