Dartford MODEL Primary Care Network is seeking an enthusiastic and forward-thinking GP to join our team.This role will provide you withthe opportunity to work across our 3 practices; The Wellcome Practice, LowfieldMedical Centre and Dr C J Shimmins & Partners, within Dartford, North Kent.Our practices are dedicated to supporting the growth of both trainees and experienced clinical staff, with two of our sites operating as GP training practices.
We have successfully supported GPs through the New to Practice programme, and have had GPs complete various fellowship programmes.Our team of current GPs brings a variety of specialist interests, including womens health, minor surgery, and medical leadership.
Main duties of the job As a GPwithin Dartford MODEL Primary Care Network, you will be required to work as an autonomouspractitioner, responsible for the provision of medical services to the practicepopulation, delivering an excellent standard of clinical care whilst complyingwith the GMS contract.
Furthermore, the post-holder will adhere to the GMCstandards for good medical practice.Key responsibilities include:Provide a full range of medical services as defined in the core GMScontract, additional and enhanced services where appropriate.
Conductingroutine and urgent appointments, both face-to-face and via telephone.Prescribing according to local and national guidelines.Handling home visits for routine and emergency cases.Managing administrative tasks, such as letters, results,referrals, and emails.Making appropriate referrals and managing related admin.Checking and signing repeat prescriptions.Participating in on-call/duty sessions as per pro-rata hours.Supporting Extended Hours services, enhancing patient access.Collaborating with other health and social care providers todeliver integrated care.Maintaining accurate consultation records using the EMISclinical syste Offeringclinical support to colleagues, helping the team meet high standards in patientcare and QOF/Enhanced Services targets.

Job description Job responsibilities Clinicalresponsibilities:The post-holder will be required to undertake avariety of duties including surgery consultations, telephone consultations andqueries, visiting patients at home, checking and signing/authorising repeatprescriptions and dealing with queries, paperwork and correspondence in atimely fashion.Makingprofessional, autonomous decisions in relation to presenting problems, whetherself-referred or referred from other health care workers within theorganisation.Assessing thehealth care needs of patients with undifferentiated and undiagnosed problems.Screening patientsfor disease risk factors and early signs of illness in consultation withpatients and in line with current practice disease management protocols,developing care plans for health.Admitting ordischarging patients to and from the caseload and referring to other careproviders as appropriate.Recording clear andconcise consultation notesCollecting data foraudit purposes.Prescribing in accordancewith local formulary and national guidelinesThe post-holder may be required to visit patients during surgery time in an emergency ifclinically indicated.The post-holder maybe required to support enhanced access sessions as per the needs of the PCN.CommunicationUtilise anddemonstrate sensitive communication styles, to ensure patients are fullyinformed and consent to treatment.Recognise the needfor alternative methods of communication to overcome different levels ofunderstanding, cultural background and preferred ways of communicating.Delivering a quality servicePrioritise,organise and manage own workload in a manner that maintains and promotesquality, including participating in shared learning across the practice, PCNand wider organisation.

Person Specification Qualifications Essential Newly qualified GP - within 2 years and not having had a substantive post.
Completed MRCGP Full GMC registration and license to practice Eligibility to practice in the UK independently Ensure Continuous Professional Development is maintained Experience Essential Experience of working in primary care Experience using a primary care IT system such as EMIS or System One Experience of QOF and clinical audit Experience of dealing with a range of clinical conditions General understanding of GMS contract Desirable Experience of providing home visits Experience using EMIS Knowledge and Skills Essential Ability to work within own scope of practice and understanding when to refer to GPs Ability to work autonomously, and as part of a multi-disciplinary team Ability to evaluate the safety and effectiveness of own clinical practice Understanding of evidence-based practice Ability to effectively triage, assess and diagnose Ability to request and review pathology tests and other relevant investiagations and advise patients accordingly Assessing and managing acute deteriorations/exacerbations of LTCs Understanding of evidence-based practice Ability to assess and manage patient risk effectively and safely.
Broad knowledge of clinical governance Excellent communication skills (written and oral) including recording of accurate clinical notes Strong IT skills incl.
