Job summary We are looking for an enthusiastic, motivated Practice Nurse withleadership skills to join our friendly, supportive team.
The ideal candidatewill have experience in chronic disease management and a strong commitment topatient care.

Main duties of the job Key Responsibilities: Chronic Disease Management (Diabetes, CHD, Asthma/COPD) Women's Health, including cytology Vaccinations and childhood immunisations Leadership and mentorship of the nursing team Contribution to service development within the practice Working closely with GPs and other healthcare professionals About us Low Moor Medical Practice is a modern,purpose-built practice with a team of dedicated healthcare professionalsproviding high-quality care to 10200 patients.
We are actively involved withour local LMC and are proud to be a high-achieving practice with consistentlyexcellent QOF results.
Our facilities include enhanced services like Level 2Diabetes and attached services such as physiotherapy and ultrasound.We have a strong, supportive team, includingfour GP partners, three salaried GP, three Advanced Nurse Practitioners, and aNurse Prescriber, as well as daily in-house pharmacist support.
Our practicealso provides training for medical students, creating a dynamic learningenvironment.
Job description Job responsibilities Assist in and perform routine tasks related topatient care as directed by clinical leads and GPs.
Such tasks include:Conduct chronic diseasemanagement clinics, including diabetes, asthma, COPD, and hypertension.Perform cervical cytology (smeartests) and provide sexual health advice.Administer vaccinations andimmunisations, including childhood and travel vaccines.Provide wound care, includingdressings and suture removal.Conduct health checks andscreenings.UrinalysisSpirometryPhlebotomyEar IrrigationChaperoning and assistingpatients where appropriate who are being examined by another clinicianRequesting pathology tests longterm condition managementFollowing agreed clinical protocolsas appropriateProcessing of samplesOther taskswill include:Clean and sterilise equipment asappropriateMaintaining stock and rotatingitems as requiredParticipation in healthcampaigns, flu campaignMaintain general tidiness andcleanliness of nurses roomsMaintaining and ordering healthpromotion literatureAt all times there will be aneed to maintain accurate recordsOrdering of vaccination tomaintain stock levelsParticipate in administrativesystems in the PracticeA duty to advise clinical leadsand the Practice Manager of potential problems or errors within range ofassigned tasksAttend and participate in anyPractice meetings when requiredAny other delegated dutiesappropriate to the postUndertake annual training anddemonstrate competency in anaphylaxis and resuscitation techniquesSpecialrequirements of the post:An understanding, acceptance andadherence to the need for strict confidentialityAbility to use own judgement,resourcefulness and common senseA commitment to maintain a highprofessional standard of nursing care and keep up to date with all aspects ofnursing care relevant to the postA commitment to ensure allHealth and Safety requirements and Infection Control measures are met and toreport any problems to the Practice ManagerA commitment to the effectiveuse of practice and NHS resourcesAn awareness of own limitationsand experienceTo work in accordance with the NMCCode of Conduct and within the scope of professional practiceTo maintain revalidationportfolio as a requirement of registration with the NMCCooperate with annual appraisalmeetings and practice development Person Specification Qualifications Essential Registered Nurse RN with current NMC registration.
Competence in chronic disease management, wound care, and immunisations.
Strong organisational and time-management skills.
Ability to work independently and as part of a multidisciplinary team.
Excellent communication and interpersonal skills.
Desirable Nurse Prescriber desirable but not essential.
Experience in a General Practice or primary care setting is desirable Experience Essential Experience in general practice nursing.
Proficiency in managing chronic diseases.
Women's health expertise, including cytology.
Experience in vaccinations and childhood immunisations.
Strong leadership and team-working abilities.
Desirable Experience of QOF and clinical audit.
IT proficiency and familiarity with SystmOne.
Commitment to ongoing professional development.
Polite and confident, motivated.
