Frailty Nurse Practitioner, Band 6

Job summary An exciting opportunity has arisen for an enthusiastic individual to join our Frailty Assessment Service as a band 6 Nurse Practitioner.
The role involves completing a Comprehensive Geriatric Assessment to those individuals attending the Emergency Department and Frailty Assessment Unit working as part of a multidisciplinary team to achieve a positive outcome for the patients.
The successful candidate would need to work within this fast paced environment, with support from a team of COTE consultants in order to make swift decisions regarding admission avoidance, taking a positive risk taking approach.Additional contract details: End date- week ending 15th March 2026Additional working pattern details: Including weekends and bank holidays Main duties of the job To undertake complex clinical assessments with the support of the consultants and wider multidisciplinary team, to evaluate and make clinical decisions regarding patient care and discharge pathway.
The candidate would have excellent communication skills and liaise with the multidisciplinary team, patient relatives and primary care partners.
The candidate would be required to work towards completing a physical assessment qualification.
The post holder would need to demonstrate a flexible approach as the demands of the role may change depending on the demands of the service.

Job description Job responsibilities - Undertake with support of the multidisciplinary team a comprehensive geriatric assessment taking a medical history making a diagnosis, plan/evaluate care and interventions making recommendations for appropriate and timely discharge/transfer from hospital through communicating effectively with primary care and the independent sector.
- To communicate and actively participate in any multi-disciplinary team (MDT) meetings to aid patient care, treatment sessions - To communicate effectively with patients and carers, potentially complex, sensitive or distressing information in order to provide patient centred care.
- Ability to disseminate contentious and delicate information appropriately explanation of why intervention is needed, palliative care - To lead on and deliver safe care within a challenging and changing clinical environment, using equipment and resources safely as appropriate - Be a visible and accessible presence throughout the service acting as a role model to other care professionals to support them in managing complex interventions using a variety of approaches to decision making.
- To adhere to all trust policies and procedures and professional code of conduct.
- To administer medication and treatment following the Trust Procedure for Ordering, Prescribing and Administration of Medical Guidelines (POPAM) administration of intravenous medication when required - Maintain accurate and contemporaneous records.
To input and retrieve patient information from computer based systems in accordance with the data protection act.
- Maintaining patient and public expectations of the service by working towards meeting local and national health targets - To support the clinical area ensuring adherence to protocols, policies and procedures.
Assist with service delivery to the agreed standard, meeting organizational and national targets - Develop and maintain clinical skills and knowledge necessary to deliver an evidence-based service.
- Be willing to work towards completion of Physical Assessment and Clinical Reasoning for Practice short course (PACR).
- Participate in mentorship, supervision and educational programmes for students and other junior members of the team.
