It was launched in April 2020 following a period of development and integrates primary care with community services within the Dudley healthcare system.
Our core aim is Dudley First, community where possible and hospital where necessary, and our purpose is to improve the health and wellbeing of our local population in Dudley.The primary focus of the post holder will be to support high quality service delivery of the First Contact Mental Health Service across a range of practices within their designated PCN to the population of Dudley.
The post holder will be required to engage a range of key partners and stakeholders and lead on joint working across health and social care.
In particular, they will need to work closely with colleagues within DIHC, other provider organisations, local authority/Office of Public Health, and the voluntary and community sector.
Main duties of the job For an extensive list of main duties please refer to the attached job description however to provide insight to this role an applicant could be expected to: To take a leading role in the day to day delivery of a high quality First Contact Mental Health service, managing resources and conflicting priorities.Undertake clinical duties as an autonomous Mental Health practitioner, offering a comprehensive assessment (including risk assessment) without the need to see a GP first, short-term mental health interventions, providing a medication review and prescribing advice where appropriate, on-wards referral to the most appropriate service for people who require longer-term mental health intervention, seeking medical support when appropriate.Be an integral part of the general practice team and provide mental health assessment and care planning for patients within the designated PCN to meet the patient's needs.Provide support and advice to GP's and other practice staff (including relevant prescribing) in managing the mental health of the population About us DIHC is committed to embracing diversity and we recognise the need to ensure our workforce is representative of the communities we serve to enable us to achieve our purpose and commitments.We recognise that we are under-represented in a number of areas, and therefore want to positively encourage applications from individuals from black, Asian or other ethnically diverse groups, people with disabilities and people from the LGBT community.We are embedding values based and inclusive recruitment practices to ensure that all applicants, from any backgrounds, have an equal chance of success in achieving a role with us.Flexible working options can also be discussed in more detail with the Recruiting Manager for the post, or line manager once in post as part of our Flexible Working Policy.

Person Specification Education and Qualifications Essential Registered Mental Health Nurse (RMN) Current registration with NMC Evidence of continuous professional development Advanced health assessment (or working towards) Non-medical prescriber (or working towards) Desirable Brief psychosocial interventions (counselling skills, CBA approach) Experience Essential Minimum 5 years post registration experience Ability to work as an autonomous and independent practitioner Experience of working in a primary / community and secondary care environment Experience of working as part of a multi - disciplinary team Experience of working with individuals with complex mental health needs both acute and long term Desirable Experience of working in a specialist service Clinical knowledge and skills Essential Experience of dealing with highly sensitive, highly complex and emotive information Advanced understanding of mental health problems Experience and evidence of well-developed comprehensive mental health assessment skills Ability to develop care and treatment plans with patients and their significant others Ability to undertake mental health interventions and evaluate their effectiveness Evidence of well-developed risk assessment and management skills Good understanding of mental health services across primary and secondary care and the voluntary sector
