Main duties of the job To succeed in this role, you need:To hold QCF Diploma or NVQ Level 2 in relevant area willing to obtain Level 3 Experience of working in Care A driving license and use of personal vehicle for work purposes To be confident travelling around the London Borough of Barking & Dagenham An enhanced DBS Check this can be arranged through ECL if needed Exceptional communication skills The ability to build and develop strong relationships with stakeholders Solid IT skills The ability and confidence to work independently and manage your own time A proactive, adaptable and customer focussed work ethicFull induction and training provided with ongoing support.

Experience Essential Substantial experience of working within a care environment.
Computer literate and a good eye for detail to ensure customer records are accurate and up-to-date.
Proactive be able to forge relationships with health and community practitioners as well as finding referrals.
Networking and forging relationships to gain a full understanding of the Community and what offerings are available.
Understands the importance of Health & Safety and has experience with Manual Handling.
Experience with dealing with sensitive data and understanding of Information Governance.
Ability to interpret community care assessments into Service User plans.
Experience in or ability to learn and use IT systems used in the service.
Ability to undertake role-based risk assessments
