Family Partnership Practice Lead

Job summary This is an exciting opportunity to join the Kent Health Visiting Service as a Family Partnership Practice Lead(FPPL) to work within the Thanet District.
The FPPL has a key role in embedding the Family Partnership Modelwithin Kent Health Visiting Services under the guidance of the Family Partnership Programme Manager.
TheFamily Partnership Model is an innovative approach which is evidence based and enables parents and familiesto overcome their difficulties, build strengths and resilience and fulfil their goals more effectively.
Main duties of the job Working as part of the local Health Visiting team, the Family Partnership Practice Lead will facilitate the earlyidentification of vulnerable families and support the delivery of the KCHFT Family Partnership Programme bymanaging a small caseload of families, supporting them ante-natally and until their infant is a year post holder will also have a lead role in embedding the Family Partnership Model within district healthvisiting practice as well as providing leadership and some line management responsibilities to the work with clients is intensive and challenging but provides a real opportunity to make a difference to theirlives.
You need to be a resilient and flexible Health Visitor with excellent communication and team leadershipskills.

Person Specification Qualifications Essential Nursing or midwifery qualification and registered with the Nursing and Midwifery Council Specialist Community Public Health Nurse qualification Degree level or equivalent professional qualification Willingness to undertake specialist training to enable the delivery of the Family Partnership model Mentorship qualification Desirable Educated to Masters level Teaching qualification Clinical supervisor Experience Essential o A minimum of 5 years' experience within the NHS or statutory sector organisation.
o Significant experience of working with vulnerable families o Significant experience of midwifery, child health or public health o Experience of working with children and families using the Healthy Child Programme o Experience in managing highly complex, sensitive information to include analysis of data.
o Good experience of multi-agency partnership working.
Desirable Experience of conducting formal project evaluation and or research.
Experience of undertaking root cause analysis Training and experience of conducting investigations Special knowledge and Expertise Essential o Knowledge and application of theories of behaviour change and motivational interviewing.
o National and local safeguarding policies and procedures.
o Specialist knowledge in relation to the delivery of the Healthy Child Programme.
