Job summary Are you a dynamic, enthusiastic, forward thinking Extended Scope Nurse or Paramedic in Frailty/Palliative Care looking for a new venture with a like-minded clinical team?
If so, HRH are looking to employ a full-time Extended Scope Nurse or Paramedic who has a passion for high quality, patient-led care and a desire to make a difference.Benefits:Flexible working patterns Access to EAP Access to the NHS pension Main duties of the job To provide ahigh-quality, cost-effective service to the patients of the Practice; focusingon frailty and palliative care.You will beworking as part of a multi-disciplinary team which include, GP Partners, GPs,Advanced Nurse Practitioners, Nurse Practitioners, Paramedic Practitioners,Clinical Pharmacists, Practice Nurses and HealthcareAssistants.You will beresponsible for home visits, face to face clinics and telephone consultationswith patients presenting with a wide range of minor illness, urgent and same dayrequests for treatment and chronic disease management, providing care for thepresenting patient from initial history taking, clinical assessment, diagnosis,treatment and evaluation of care.You will needto demonstrate safe, clinical decision-making and expert care, including physicalassessment and diagnostic skills.
You will need to prioritise and triage theneeds of patients accordingly making any necessary referrals for investigationsin the appropriate manner.The role willinclude holistic assessment, support and coordination of care for Palliativeand End of Life patients.The role willbe involved the care of frail and elderly patients including the responsibilityof patients within care and nursing homes.This will include weekly ward rounds.

Part of your responsibilities willinclude visiting patients in their home to assess their medical condition,frailty review and may include those with low-level mental health concerns Person Specification Qualifications Essential Full and unrestricted NMC (UK) or HCPC RGN or Paramedic (degree or equivalent) Desirable Prescribing Qualification A qualification in Frailty Care Qualification in Physical Assessment skills Experience Essential To have experience and knowledge of a wide range of chronic health conditions common in elderly or frail patients such as Cardiovascular disease including Hypertension, IHD, stroke, Heart Failure etc, Respiratory disease such as COPD, Diabetes, Dementia, Atrial Fibrillation etc.
Experience of working with frail and palliative patients and those at the end of their life.
Experience and knowledge in physically assessing patients Experience of providing specialised advice in relation to care Desirable Previous Experience in General Practice Knowledge and Skills Essential Evidence of continuing professional development Evidence of clinical knowledge in caring for patients in relevant specialism In-depth knowledge of recent NHS legislation, recommendations for specialist area.
