We're seeking an experienced Social Worker to join our Hospital Discharge Team, focusing on innovative front-door hospital approaches for swift discharge pathways.Positioned in three acute hospitals, our teams provide crucial advice, assessments, and discharge planning, ensuring safe and timely transfers of care through a collaborative, multi-disciplinary approach.
As a key team member, you'll conduct complex assessments, serve as an Investigating Officer in safeguarding cases, and apply expertise in the Mental Capacity Act 2005 and the Care Act 2014.Bring at least 2 years of post-qualifying experience and Social Work England registration, confidently offering peer supervision and support to fellow social workers and customer coordinators.
