The MSK physiotherapy servicehas worked closely with County Durham CCG to commission anintegrated MSK service for CountyDurham patients (CD IMS).
The CD IMS pathway provides asingle point of access to the fullspectrum of MSK services from routine physiotherapy tospecialist physiotherapy and orthopaedics.
Main duties of the job All patients referred will be triaged by an ESP, who willdirect patients to the right clinician, firsttime.
This may include physiotherapy, ESP opinion fordiagnostics, onward secondary care referralor AHP referral.As an ESP, you are responsible for delivering a leadingstandard of musculoskeletal physiotherapy assessment, triage and care to a varied selection of NHSpatients.
You will work as a Frontline ESPin various locations, as part of a wider integratedcommunity physiotherapy service.
You will be required to perform frontline patient screening with directreferrals from GPs, care navigation andself-referrals; using expert assessment and diagnosis todetermine which clinical pathway will bemost suitable for the patient.
You will also be required toprovide expert clinical assessment,diagnosis and management of your caseload.From initial assessment you will be responsible andaccountable for devising, reviewing andcompleting appropriate treatment management programs to thepatient population, in order togain successful clinical outcomes, in line withyour regulatory bodys standards of professional practice.
