Job summary The Diabetic Enhanced Nurse will provide specialist care andsupport to patients with diabetes as part of Preston Park PCN.The role involves managing complex diabetic cases, providingeducation, and promoting self-management to improve patient outcomes.
Workingclosely with the wider primary care team, the Diabetic Enhanced Nurse willensure that high-quality, evidence-based care is delivered, following bestpractices in diabetes management.
Main duties of the job 1.Lead in the management of patients with Type 1, Type 2 and complexdiabetes, including those requiring insulin therapy or complex regimes.
2.Conduct comprehensive diabetes assessments, including glycaemic control,cardiovascular risk management and complications screening.
3.Develop and implement individualised care plans to optimise blood sugarcontrol, reduce complications and improve quality of life.
About us Our Primary Care NetworkPreston Park PCN is a group of five surgeries - Beaconsfield Medical Practice, The Haven Practice, Preston Park Surgery, Stanford Medical Centre and Warmdene Surgery.
We serve approximately 57,000 patients and aim to support and connect with our local community.Primary Care Networks (PCNs) bringGP practices together with other local services such as community, mental health, social care and the voluntary sector to look after local populations.
PCNs will help to joinupservices at a local level, focusing on the specific needs of these local populations, with patients still accessing routine GP appointments as they do now.What are Primary Care Networks?PCNs build oncurrent primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care.
Clinicians describe this as a change from reactively providing appointments to proactively providing care for the people and communities they serve.
Where emerging PCNs are in place in parts of the country, there are clear benefits for patients and clinicians.
PCNs usually have a 30-50k population and are geographically contiguous (next to or touching another).

The role involves managing complex diabetic cases, providing education, and promoting self-management to improve patient outcomes.
Working closely with the wider primary care team, the Diabetic Enhanced Nurse will ensure that high-quality, evidence-based care is delivered, following best practices in diabetes management.Please ensure you meet the essential criteria and are level 7 qualified.
Core Hours Hours Per Week, part time hours are also available Key Responsibilities 1.
Lead in the management of patients with Type 1, Type 2 and complex diabetes, including those requiring insulin therapy or complex regimes.
2.
Conduct comprehensive diabetes assessments, including glycaemic control, cardiovascular risk management and complications screening.
3.
Develop and implement individualised care plans to optimise blood sugar control, reduce complications and improve quality of life.
4.
Educate patients and their families/carers on diabetes management, including diet, exercise, medication adherence, and monitoring.
5.
Provide training in blood glucose monitoring, insulin administration, and understanding the role of diet in diabetes control.
6.
Support patients in achieving lifestyle changes to manage their condition effectively, focusing on long-term outcomes.
7.
Serve as the clinical expert within the practice and the ARRS team, providing training and support to other healthcare professionals.
8.
Ensuring the implementation of the latest clinical guidelines and best practices.
9.
Provide expert guidance to the wider primary care team on the management of diabetic patients.
10.
Participate in clinical meetings and contribute to case discussion on complex diabetic care.
11.
Collaborate with GPs, pharmacists, dietitians, and other healthcare professionals to deliver coordinated care for diabetic patients.
12.
Liaise with secondary care specialists, such as endocrinologists and diabetes clinics, to ensure seamless transitions of care for patients requiring advanced management.
13.
Lead or participate in multidisciplinary case reviews for complex diabetic patients.
14.
Contribute to the ongoing development of diabetes services within the practice, identifying areas for improvement and implementing changes to enhance patient outcomes.
15.
Participate in audits and data collection to monitor clinical performance and outcomes in diabetes management.
16.
Help the practice meet national diabetes targets, such as QOF (Quality and Outcomes Framework) and local commissioning requirements.
17.
Maintain accurate, detailed clinical records of patient assessments, treatments, and outcomes in line with practice protocols.
18.
Develop comprehensive diabetes care plans that are personalised to each patient, reviewing and adjusting them regularly.
19.
Ensure patient care plans align with overall treatment goals, including cardiovascular health, weight management, and lifestyle modification.
20.
Provide training and education to other healthcare professionals within the practices on diabetes management, ensuring they are up to date with current best practices.
21.
Actively participate in your own professional development, attending relevant training sessions, conferences, and maintaining clinical knowledge through continuous professional development (CPD).
22.
Conduct home visits for housebound diabetic patients when necessary to assess their condition and provide care.
23.
Utilise remote monitoring tools, including digital health technologies, to support patients in managing their diabetes effectively at home.
24.
Contribute to the development and review of diabetes care protocols within the practice to ensure compliance with local and national guidelines.
25.
Identify patients at risk of diabetes complications, such as foot disease, cardiovascular disease, or diabetic nephropathy, and take proactive measures to reduce these risks.
26.
Participate in clinical governance activities, ensuring that the diabetes service meets regulatory standards and promotes patient safety and care quality.

Essential Skills/Knowledge/Experience Registered Nurse with valid NMC registration Postgraduate certification or diploma at Level 7 in Diabetes Care Extensive experience in managing diabetes care, including complex cases such as insulin initiating and adjustment.
Strong understanding of diabetes pathophysiology, treatment options, and lifestyle interventions for managing the condition.
Excellent communication skills to educate and support patients, families, and other healthcare professionals.
Ability to work independently and as part of a multidisciplinary team in both primary and secondary care settings.
Experience in primary care or community settings.
