Job summary The Integrated Transfer of Care Hub (ITOC) have an excitingnew role, we are looking to recruit a Discharge Support Officer to workalongside the Multidisciplinary Team.
You will have delegated responsibilityfor facilitating the delivery of safe, effective, and timely discharge/transferof patients from Mid Yorkshire Teaching Hospitals into the community or other hospital/communityfacilities.
These maybe both simple andcomplex.
The post holder willbe able to make judgement involving facts or situation which may requireanalysis alter programmes of activities or making decisions related tospecific pathway of care, whilst acting as a central point of contact fordischarge planning.
We are looking for an energetic individual who has a soundbackground of the health and social care environment to coordinate dischargesand work within the multi-disciplinary team.This is a seven-day service that provides single point ofcontact across all wards and hospital sites within Mid Yorkshire Teaching HospitalsTrust for staff, patients, families, carers and associated people andco-ordinate / contribute to the safe and timely hospital discharge in partnershipwith other multi-disciplinary colleagues.Please note that this role is not eligible forsponsorship under the Skilled Worker route.Please note that this role is advertised at Band 4 but is pending confrimation of banding.
Main duties of the job Main Duties of the JobTo line manage a small team of Administrative Staff with in ITOC.To work with the discharge sisters and dischargecoordinator to promote the correct implementation of discharge processesthroughout Mid Yorkshire hospitals to ensure safe and timely discharge andsmooth transfer of patients.Actively participate in discharge planning andfeel confident in ensuring that discharge activity is occurring.Provide concise and detailed information regardingpatients discharge plans.Work closely with all internal and externalpartners and agencies to ensure a seamless service, including working to reducedelayed discharges.To promote best practice in achieving highquality discharge planning and support in reducing No Reason to Reside numbers.
To use tact and diplomacy where there is disputeover complex issues regarding leaving hospital and there may be disputesregarding courses of action to be taken.To liaise with relevant staff and agencies on adaily basis to resolve daily operational issues and ensure discharges aremanaged effectively.Acts as an advocate for patients and theirfamilies/carers recognising the boundaries of their clinical knowledge;liaising and referring on to other services / agencies as required.To understand and apply the skills and knowledgearound complex needs and discharge planning.

To apply knowledge of the effect of disabilityand provide advice on discharge planning processes, options/ services availableto facilitate discharge and the roles and requirements of the multidisciplinaryteams.
To progress chase each patient through theirhospital stay ensuring diagnostics and results aredelivered and reviewed in a timely manner.To use knowledge, training and experience toanalyse, plan, and implement discharge plans for patients with complexphysical, mental health and social needs.Using knowledge and understanding to supportstaff from across agencies in planning effective discharges.To be able to use problem solving skills, todetermine appropriate actions and formulate specific and appropriate managementplans.
To have a working knowledge of currentlegislation, National Service Frameworks, clinical standards and nationalpolicies in order to provide an effective discharge service.
Person Specification Qualifications Essential Educated at diploma level or equivalent.
Relevant IT skills to include, E-mail, Excel, Window Word (equivalent to RSA 3, European Driving Licence) Skills and Abilities Essential Able to use complex communication skills in a variety of settings, including management of confidential and sensitive information.
Be able to analyse and evaluate the information collected and formulate appropriate recommendations.
Be able to problem solve complex situations and recommend appropriate action.
Good negotiation skills including the ability to diffuse difficult situations and overcome barrier to understanding.
Judgement involving complex situations particularly where there are conflicting information to determine and resolve Experience Essential Previous experience of working with people in an acute hospital setting Previous experience of Public Sector work Ability to work autonomously to achieve required targets Experience or understanding of patient flow and discharge planning, Able to establish and maintain a safe practice environment for self and others.
