Job summary The Integrated Transfer of Care Hub are looking torecruit Discharge Co-ordinators (previous applicants need not to apply) to work on the wards across the acute hospitalto increase patient flow and optimise patient experience surrounding alltransfers of care.We are looking for energetic and caring people who have anunderstanding and some knowledge of the health and social care environment tocoordinate discharges and work within the multi-disciplinary team.This is a seven day service that provides single point ofcontact across all wards and hospital sites within Mid Yorkshire Hospitals Trust for staff, patients, families, carers and associated peopleand co-ordinate / contribute to the safe and timely hospital discharge inpartnership with other multi-disciplinary colleagues.Please note that this role is not eligible for sponsorship under the Skilled Worker route Main duties of the job To promote the correct implementation of dischargeprocesses throughout Mid Yorkshire hospitals to ensure safe and timelydischarge and smooth transfer of patients.
Actively participate in the ward based dischargeactivity ( SAFER, EDDs, board rounds and the identification of patientsfor transfers to PMU and DDH) to ensure thatdischarge activity is occurring for every patient within the ward.
To maintain momentum of discharge planningthroughout the entire process.
Provide concise and detailedinformation/handover regarding patients discharge plans.
Work closely with all internal and externalpartners and agencies to ensure a seamless service, including working to reducedelayed discharges.
To link closely with clinical Site managers toensure effective use of beds.
To promote best practice in achieving highquality discharge planning To be able to use problem solving skills, todetermine appropriate actions and formulate specific and appropriate managementplans.

By applyingknowledge of skill deficits, components of self-care, productivity and leisureto ensure that comprehensive holistic assessments have been carried out toenable a smooth and effective discharge.To apply knowledge of the effect of disabilityand provide advice on discharge planning processes, options/ services availableto facilitate discharge and the roles and requirements of the multidisciplinaryteams.
To use knowledge, training and experience toanalyse, plan, and implement discharge plans for patients with complexphysical, mental health and social needs.Using knowledge and understanding to supportstaff from across agencies in planning effective discharges.To be able to use problem solving skills, todetermine appropriate actions and formulate specific and appropriate managementplans.To have a working knowledge of currentlegislation, National Service Frameworks, clinical standards and nationalpolicies in order to provide an effective discharge service.
