Diabetes Technology Specialist Nurse

Job summary As an expert Practitioner, the post holder will practice autonomously and also within a defined multi-disciplinary team to provide advice and support to patients, carers, and healthcare professionals on technology/insulin pumps and Continuous Glucose monitoring systems, across both secondary and primary care and in accordance with local guidelines and policies.
The post holder will work closely with the diabetes clinical leads and Senior nurse to support the effective clinical strategy and operational management for the insulin pump and CGM services, ensuring a high-quality patient care within the resources available.
To provide effective leadership and management of the Diabetes technology service across ABUHB.
The post holder will be integral to the development and improvement of the technology service in collaboration with the diabetes team.
Main duties of the job Contribute to and advise on the strategic development of Integrated Diabetes Services and be responsible for the management of the operational delivery of specialist nurse led diabetes services across ABUHB.
To improve patient care and outcomes in conjunction with team members by being responsible for the Pump service improvement, identifying areas using transformation skills, benchmarking, best practice, audit and research to ensure services are effective and sustainable Perform specialist assessment of patient's clinical needs, plan, implement and evaluate care delivery using clinical knowledge and skills for patients who will be managed by insulin pumps and CGM.
Support patients and their carers, ensuring they receive the required information to enable them to self-manage their care delivery.
Coordinate and implement nurse led outpatient service for patients on insulin pumps in collaboration with diabetes MDT The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
