Job summary We are looking for an ambitious, dynamic, motivated and enthusiastic Band 6 registered nurse qualified or with a specialist interest in Diabetes, to join our Community Nursing Service in Bedfordshire.
This fixed term post has been created in order support the Community Nursing teams reduce and manage the volume of patients being visited for insulin management.
Main duties of the job The primary focus of this post will be to support the education and promotion of self-management, including the review of current insulin regimes and possible onward referral for diabetes management.
You will be a strong leader, focused on achieving real improvements in outcomes for patients and will be confident in your ability to support, coach and develop patients and teams to deliver self-care in the management of diabetes.
You will be innovative, forward thinking, self-driven with excellent communication and interpersonal skills.
You will take pride in your work and be highly motivated and dynamic, have a flexible approach and be able to embrace change.
Being a collaborative worker and the ability to network and build relationships with patients and stakeholders is essential.
