Diabetes Specialist Nurse – Community Diabetes Service

Job Title: Diabetes Specialist Nurse – Community Diabetes Service Tripod Partners are currently recruiting for a Band 6 Diabetes Specialist Nurse to join the Community Diabetes Service in Peterborough.
This is a full-time role, Monday to Friday, with the responsibility of managing a caseload of patients with complex diabetes needs.
Key Responsibilities Work as part of a multidisciplinary team to provide ongoing care for patients with diabetes, aiming to prevent hospital admissions and manage care in the community.
Manage a caseload of patients with complex diabetes needs, using evidence-based practice and patient-centred goals to assess, plan, implement, and evaluate interventions.
Promote a better understanding of diabetes for patients, their families, carers, and other agencies involved through education, support, and counselling.
Collaborate with the multidisciplinary team to ensure comprehensive care and support for patients.
