The role has become available to join a committed and motivated Patient Flow Team who work hard to deliver excellent standards in a positive working environment.
The Hospital Assessment Team works in partnership with individuals to identify what matters to them.
This includes undertaking Mental Capacity Assessments, Continuing Health Care Assessments, leading Best Interest Meetings to ensure people have a safe and timely discharge from hospital.
The post holder will be expected to supervise and support team members in carrying out tasks, to report to the team manager and senior management regarding the progress of the team.
To manage more complex cases and deputise for the Team Manager when required.
The team work alongside a range of professionals, including colleagues from the health board, therapies and external providers.

Ideally you will have;Applicants will need to be social work qualified and professionally registered as a social worker with Social Care Wales.
You will need a knowledge and working practice of the Social Services and Wellbeing (Wales) Act 2014, to include outcome focussed assessment and care planning, knowledge and working practice of Continuing Health Care, the Mental Capacity Act 2005 including Deprivation of Liberty Safeguards.
