Job summary We have an exciting opportunity to welcome a Deputy Community Team Lead to our Community Nurse Specialist team.
The role is based within Yeovil and covers the South Somerset area.You will undertake the holistic assessment, planning and delivery of specialist palliative and supportive care for patients, with advanced, progressive life limiting disease.You will join us on a full time, permanent basis and in return, you will receive a competitive salary of £46,468 per annum plus 30% Unsocial Hours Enhancement on Saturdays and 60% on Sunday & bank holidays Main duties of the job Working closely with the countywide community team lead, your day to day work will include:Managing aspects of the service, including clinical assessments, triaging, care planning, clinical supervision,mentoring, and assessing workload/service activity.
Assessing the needs of the patients and families on caseloads and co-ordinating appropriately.
Acting as a role model, leading and engaging the team through change and development Assessing patient needs, identifying their goals and working with the patient and family towards achieving them.
Making referrals to the multi-disciplinary team and work collaboratively with a wide range of community-based services.This role requires working weekends and bank holidays on a rota basis; therefore, flexibility and teamwork are crucial in providing this service.

As Deputy LeadNurse you will be required to provide a positive approach to change managementand demonstrate resilience in leadership.
You will also be required to play anactive part in the delivery of the Clinical Strategy and hold insight andcreativity as additional skills.
The Clinical Nurse Specialist (CNS),withinthe community team, is a specialist practitioner with extensive knowledge,skills and competency to manage a caseload of patients with advanced,progressive life limiting disease.The CNS will undertake the holistic assessment,planning and delivery of a consistently high standard of specialist palliativeand supportive care in accordance with the hospice policy, procedures andstandards.
The CNS will establish effective working relationships with widermulti-disciplinary teams and act as a resource for professionals and non-professionals involved in palliative care within the community.
Person Specification Education and Qualifications Essential Registered Nurse RN1 Degree level or equivalent study in related subject, such as acute oncology, palliative care, chronic conditions management (or willing to work towards) Post registration qualification or experience in palliative care/oncology or related subject Recognised teaching qualification or prepared to work towards Evidence of leading a team at a senior level Desirable Non-medical prescribing qualification Clinical Examination Skills for Advanced Practice Physical Assessment and Clinical Reasoning Management qualification Self Awareness Essential Calm and objective Assertive, confident yet approachable Personally and professional mature Recognises own limitations Ability to lead and influence change Desirable N/A Working with others Essential Recent leadership and management experience.
Experience of multi-professional working Desirable N/A Outcome and Results Essential Relevant post-registration experience in cancer, palliative care or relevant speciality In-depth specialist experience in care of patients with complex needs.
Computer literate, able to use word documents, database and e-mail communication Desirable Experience of mentorship and teaching.
Experience of community nursing.
