Woodlands Care Home is seeking a Dementia Coordinator to provide compassionate care and guidance to residents and their families.
This role focuses on person-centred support, helping residents live well with dementia, while empowering them to engage in meaningful daily activities.
Key Responsibilities: Collaborate with residents and their families throughout the moving-in process, ensuring a smooth transition and a welcoming environment.
Develop tailored, person-centred support plans reflecting the individual needs, preferences, and interests of residents.
Design and coordinate well-being programmes and activities that align with residents' hobbies and routines, enhancing their daily experience.
Liaise with families to identify and implement effective strategies for managing dementia-related behaviours.
Support staff in building dementia care skills, completing documentation, and responding to stress or care refusal situations.
Organise community events, including quarterly dementia cafés, to foster peer support and share best practices.
Work closely with allied health professionals to improve residents' quality of life and help reduce reliance on psychoactive medications.
Regularly review and assess residents' needs, ensuring ongoing, high-quality care.
