Job summary Rochford District Primary Care Network (PCN) have a fantastic opportunity for a permanent data and outreach coordinator to join our team.
This position is offered on a full-time basis, working hours per week, ideally Monday to Friday and is covering the following practices: Ashingdon Medical Centre The Practice Leecon Way The Puzey Family Practice Wakering Medical Centre The role involves providing high quality, comprehensive and effective administration skills, whilst providing a central co-ordination role for patient care planning across member practices.
Main duties of the job The appropriate individual will have SystmOne and primary care experience.
You will need to be confident individual who can communicate with a ride range of professionals at all levels.
The successful applicant will have the ability to work without supervision using their own initiative, organising and prioritising own and other workloads to changing and often tight deadlines.
You will be enthusiastic and committed.
