This exciting new role is part of our Neurodiversity Transformation Programme and is building on the successful LDAP (Learning Disability and Autism Pathway) Keyworker pilot that was designed in partnership with NHS England.
The Neurodiversity Navigator role will be to support the child, young person and their families who are awaiting assessment for a diagnosis.
They will focus on the young persons needs, supporting the family to navigate the system and access local support whilst waiting for their assessment.We will continue to deliver a new way of supporting Children, Young People (CYP) and their family/carers that are having difficulties with any neurodiverse presentations.
This role will enable CYP and their family/carers to get the right support at the right time and ensure local systems are responsive to meeting their needs in a holistic and joined up way.We are looking for enthusiastic and proactive candidates who want to make a difference to the lives of Children and Young People in Devon.
We are looking to recruit multiple positions to work across education, health, social care and local communities to enable whole systems to change and embed principles of transforming care and SEND reforms to improve the experience of CYP and families/carers.These roles are being hosted by LWSW but will be locality based across the Devon foot print and posts will be working as part of the local multi agency response in Devon, Torbay and Plymouth.
Main duties of the job We are recruiting a team of Neurodiversity Navigators that will provide support across the Devon County footprint.
You will be based in locations across the county to provide local support for our families in an attempt to reach out to as many as possible.Support with navigating the complex networks of health, education, social care, community and voluntary servicesActive listeningInformed adviceConnecting people to community groups, parent/carer forums and statutory services for practical and emotional supportSupport completing formsTargeted support with simple personalised support plansAdvocate when required but not in place of other advocacy servicesThis role may not be eligible for sponsorship under the Skilled Worker route, please refer to the Direct Gov website for more information with regards to eligibility.Please note that Livewell may close the job advertisement earlier than the specified deadline if a high number of applications are submitted.

Job description Job responsibilities The principal purpose of the job is to improve the health & wellbeing of CYP and their families/carers with a range of possible neurodiverse difficulties.Active involvement in the delivery of high quality, safe and compassionate service as an effective member of the teamImplementing specific assessment, review and care packages to individuals and groups within standard protocols, and integrated care pathwaysProviding evidence-based advice and consultancy to young people, family members/carers within standard protocolsOrganising and managing a defined caseload under appropriate guidance and work as lone workers where appropriateThe post holder will be accountable for their own actions within the delivery of this care.
Seeking advice and expertise of a registered practitioner when required through the use of regular managerial and clinical supervision in accordance with Livewell policy.Communication and Working RelationshipsTo communicate in a skilled and respectful manner with young people, families and carers who may have specific difficulties in understanding and/or communicating and who may be distressed and/or present with behaviour that can be challengingTo participate in clinical meetings and to collaborate with other colleagues in the wider multi-disciplinary servicesTo maintain appropriate professional boundaries whilst recognising abilities and limitations of own competenceTo communicate complex and sensitive condition related information concerning the assessment, treatment and risk factors in relation to a range of possible neurodiverse difficultiesTo always establish and maintain clear and professional relationships with othersTo liaise and work collaboratively with a range of other professionals in statutory and non-statutory organisations agenciesProvide a high standard of written and verbal communication that is clear and in line with Trust documentation standards.Planning and OrganisationTo organise, plan and prioritise own workload under distant guidance of a registered practitionerTo provide evidence-based advice, guidance and consultancy to patients, family members, carers and professionals within standard protocolsPlanning, co-ordination and review of services including active reviews and review of placements as delegated by other registered practitionersOrganise clinical meetings and ensure that interventions/services are planned in an integrated and co-ordinated manner to best support the client/carers, this may include liaison with external agenciesAnalytical and Judgement SkillsRecognise own clinical competencies, knowledge base and experience, and ensure that you practice within this and seek support as requiredTo make informed and well-reasoned decisions, recognising when to seek further advice from a registered practitionerUndertake risk assessments and determine the appropriate course of action, which has taken into consideration a range of possible options.
Where necessary, ensure that consultation with senior colleagues is undertakenRecognise and take action to deal with any situations which may provide a conflict of interestResponsibility and AccountabilityTo comply with LW policies and procedures and the Code of Conduct for Health Care Support WorkersTo promote and uphold the privacy, dignity, rights, health and wellbeing of clients and their care givers at all timesResponsible for the delivery of specific care packages including assessing, planning, intervention and evaluation within standard protocols and accountable for their own actions within the delivery of this careProvide evidence based clinical advice in relation to the individuals in the care of the teamEnsure that appropriate action is taken for safeguarding children under the guidance of a Senior PractitionerUnder the direction of and upon successful completion of any required additional training, can assist (in accordance with NMC delegated authority guidance) a registered nurse to ensure that recommended physical health and other monitoring requirements of mental health medication (in line with relevant trust policy, standards and guidelines) is completed, recorded and if necessary escalated for further actionUnder the direction of and upon successful completion of any required additional training, can assist (in accordance with NMC delegated authority guidance) a registered nurse and /or the individual in the administration of certain topical and oral medicines as per an individuals care and treatment plan.
