Job summary Contracted Registered nurse (30 hours)Salary AFC Band 5, depending on experience (£19 per hour)An opportunity has arisen for an experienced Registered Nurse to work for HospiceCare North Northumberland.The role is pivotal in ensuring the sustainability and growth of Hospice Care services to patients and clients in North Northumberland.The post holder will be a valuable member of the clinical team, providingsupport to patients and families in their own homes who have palliative and end-of-life care needs.

We provide high-quality endof-life-care in patients homes to the communities of North Northumberland.
We also provide emotional and practical care and support to adults and their families, who are living with a life-limiting illness requiring palliative input, through our hospice at home, bereavement support, and dementia support services.
Our care is delivered free at the point of need.

Provide palliative advice andsupport to patients, families and carers., plan and implement highstandards of individualized patient care in conjunction with the Primary HealthCare Team.3.
Monitor patients condition andreport significant changes to the Hospice and Primary Health Care Team.

Be responsible for managing theprocess of receiving and responding to referrals for the Hospice at Home service.6.
Prioritising workload and managingresource allocation as required.7.
Act as a role model for new staffas they undertake their induction programme or students on placement.8.
Contribute to team meetings andhandovers.
Support H@H team with learning and development.9.
Supervise the work of HospiceSupport workers and ensure Hospice policies are followed.10.
Report incidents or emergency situations asthey arise and report them to theHead of care in line with Hospice Policies and procedures.11.
Jointresponsibility for the provision of 24-hour nursing rota when the need arisesand for professional development in the total care of patients requiringspecialist palliative care General Responsibilities undertake all mandatory training asrequired by HospiceCare andparticipate in appropriate in-servicetraining.13.
Maintain strict confidentiality of allinformation acquired especially withregard to patients and staff.14.
To undertake an appraisal andpersonal development review annually andthrough self-development, continuously updateand improve knowledge and competencies.

To use skilled communication tosupport carers and families of patients receiving services from HospiceCare andmaintain trusting effective relationships.25.
To ensure that patients andfamilies/carers are identified and appropriately referred to other servicesboth from HospiceCare and the community.26.
To contribute to the developmentand enhancement of patient care by participating in standard setting, policydevelopment and clinical audit of services.27.
Be responsible for co-ordinatingflexible care provision to enable patients to remain at home.28.
Maintain accurate patient records Learning29.

Develop own knowledge and skillsin palliative care, supported by current relevant research and contribute tobest practice development of HospiceCares hospice at home team.31.

Facilitate an environment thatoptimises and develops knowledge and skills within HospiceCares hospice athome service through reflective practice and education.
Person Specification Personal skills & knowledge Essential Excellent standard of nursing care and clinical knowledge.
High level of empathic and interpersonal communication skills.
Ability to work as part of a multi-disciplinary team.
Adaptable & able to use initiative.
Interest in effective assessment and psychosocial care of patients with complex needs and their families.
Time management.
Competent with email, internet Word and Powerpoint.
Positive & enthusiastic attitude.
Independently mobile and the ability to cover Alnwick, Berwick & the surrounding areas.
Qualifications Essential 1st Level Registered Nurse.
Accredited palliative care or oncology course or willing to undertake.
Desirable Degree in palliative care.
Experience Essential Interest in working in a palliative care setting.
Evidence of continuous professional development.
Desirable Oncology or palliative care.
Teaching and mentoring.
Audit.
