Job summary
At Yorhealth we provide care for children and young adults with complex care needs.
You will be working in the family home.
This role is to work with a baby boy who is 14 months old.
He has bronchomalacia and has a tracheotomy ventilated 24 hours a day.
He lives at home with his mum, dad, older sister and dog.
He likes playing with musical toys and watching cocomelon.
Main duties of the job
We are looking for a staff member to join this little boys team to provide three waking nights per week on a two week rolling rota.
Shifts will be set per week and we offer contracted hours to all staff.

Job description Job responsibilities MAINDUTIES AND RESPONSIBILITIES:
Supporting the family and theteam to carry out the complete range of care for the child / young person whichcan include: provision of personal care, mealtimes, medications administration,gastrostomy care, tracheostomy and ventilator care, emotional care and social/playinteraction.
Accompanying the child/ youngperson on trips out of the home to include hospital appointments and/oradmissions where required.
Contribute to a team approachto patient care in conjunction with all members of the multidisciplinary team.

Ensure full understanding ofthe Support/care Plan and communicate accurately and efficiently how thesupport/care plan is being delivered.
Participate in regular meetingsand attend other meetings as requested and required.

Attend and comply with all statutory and mandatory training requirements,this includes ensuring E-Learning is maintained and renewed as required.Attendance to all Mandatory Training sessions to maintain competence in linewith clinical requirements.
Undertake tasks and procedures,in which competency has been assessed and recorded specific to the care environment,including for example, tracheostomy and ventilation training.

Behaving in a manner that doesnot put themselves, children and young people, families, other team members orothers in danger or at risk in the work environment.
Assist in the maintenance ofstock levels, including medicines and PPE and to inform the office/clinicalteam as and when stock is needed to be reordered.
Assist in the cleanliness andtidiness of the home environment and adhere to infection control policy andprocedures at all times.
Assist the child/young person toensure they have the opportunity to meet their individual dietary requirementswhether orally or enterally fed.

Complete legible and accurate daily writtenreports on all service users in accordance with Yorhealth Policies andProcedures.
Become familiar with all organisationalpolicies and procedures and to comply with these at all times.
Maintain the confidentiality ofinformation about children, families, staff and company business and meet therequirements of the Data Protection Act (1998) at all times.
The postholdermust comply with all Information Governance policies at all times.
The work of Yorhealthis of a confidential nature and any information gained by the postholder intheir role must not be communicated to other persons except where required inthe recognised course of duty.
Be aware of theresponsibilities placed on you under the Health and Safety at Work Act (1974)and any subsequent relevant legislation and always follow these in fullincluding ensuring that they act in line with all agreed procedures at alltimes in order to maintain a safe environment.
Protect supported individualsfrom abuse and report any concerns immediately to management colleagues.
Inorder to achieve this, you are required to read Yorhealths SafeguardingPolicy, participate in regular training and incorporate preventative strategiesinto your work.
