Main duties of the job Main Duties and Responsibilities: Co-ordinate and lead Funded Nursing Care and/or Continuing Healthcare assessments or reviews for service users, liaising directly with service users/families and other key stakeholders and relevant health or social care professionals.
To complete accurate and detailed documentation, and ability to rationalise and explain any clinical recommendations made.The Nurse Assessor may be required to complete Continuing Healthcare Checklists, CHC reviews, FNC First Time Assessments or FNC Reviews, depending on the requirement for that individual.
This role may also involve reviewing the appropriateness of 1:1 care within care home environments and making recommendations to ensure that the level of care provided is appropriate to the person's health care needs.This role requires the Nurse Assessor to liaise effectively with the relevant health and social care agencies, as well as with other independent agencies, with service users and with families or representatives.To promote good practice throughout the Trust in relation to the CHC processes.Identify and promote good practice in services for people under HPFT services, in particular helping to advocate for those who may lack capacity.
