Job summary This is a substantive post for a Consultant in Critical Care and Anaesthesia.
It is a full-time post to cover sessions made available as a result of job plan changes in the department.
It is based around a 1:8 on call in Critical Care and a 10PA contract.
The position would support dual CCT in critical care.
The department would will only consider those with CCTs or equivalence in Critical Care and Anaesthesia.
Sessions will be decided upon consultation with the clinical lead and speciality director but initial approx.
2/3 of job planned sessions will be in Critical Care.
The post could be filled on a whole or part time basis.
Any candidate who is unable, for personal reasons, to work full time will still be considered for the post.
If such a person is appointed, modification of the job content will be discussed on a personal basis.
Job share applicants are also welcome.
Main duties of the job .The successful applicant will join the existing team of approx.
18 Critical Care consultants working for the trust in the two main sites of Gloucester and Cheltenham.
In addition there are approx.
45 general anaesthetic consultants working in both central sites and peripheral community hospitals.
As a senior employee of the Trust the post-holder will work in close co-operation with, and support other, clinical, medical professional and managerial colleagues in providing high quality healthcare to the Trust's patients.
Integral to the responsibilities of the post are the following requirements:- To ensure the provision of a first-class clinical service To provide effective leadership to all staff engaged in the specialty To sustain and develop teaching and research wherever appropriate To undertake all work in accordance with the Trust's procedures and operating policies To conduct clinical practice in accordance with contractual requirements and within the parameters of the Trust's and Division's services plan To maintain the confidence of business plans and development strategies formulated for the specialty, the Surgical Division or the Trust.

*Figures based CCG data on the number of people registered at a GP surgery in Gloucestershire The Surgical Division Anaesthetics, ICU and Pain Medicine are part of the Trust's Surgical Division.
The Chief of Service is Miss Margaret Coyle, who also holds clinical commitments as a maxillo-facial surgeon.
The Divisional Director of Service Delivery is Mr Simon Burchfield and the Divisional Nursing Director is Mrs. Sarah Mather (Divisional Director of Quality and Nursing Surgery).
The General Manager for Anaesthesia/Critical Care and Pain is Mrs Rebecca Offord.
Job description Job responsibilities Clinical responsibilities The post holder will provide: Undertake consultant cover for weekdays, weekday nights and weekends for Critical Care as part of the 1 in 8 rota.
Undertake daytime and weekend sessions for HDU.
Undertake up to 3-4 sessions a week in anaesthesia.
On call duties are undertaken on either or both the Gloucester or Cheltenham sites.
Support for the clinical decision making of junior medical and nursing staff.
Teaching and training To provide conditions for improved training opportunities in line with national and local recommendations.
To participate in the undergraduate teaching programme.
The Department is involved in teaching medical students from Bristol, as the Trust achieved Academy status in 2004.
Fourth year students are placed in the Department and participate in a structured teaching program.
The anaesthetic department has a close relationship with the Paramedic Training School and the local ambulance service and paramedics regularly undertake attachments.
To assist and participate with the development of postgraduate training for all grades of trainee and other staff as appropriate.
Specialist registrars have a full-day teaching session regionally once a month and sub-regional (Cheltenham or Gloucester) teaching on one afternoon a week.
The trainees are encouraged during their attachment to contribute to tutorials and presentations for the Department.
Where agreed, to act as Educational Supervisor to junior and SAS medical staff, being responsible for their appraisal and personal development planning.
Continuing professional development We recognise the importance of continuing medical education and actively encourage our consultant team to prioritise professional development.
You will undertake CME / CPD as per College recommendations.
A mentoring scheme for new consultant staff is well established and each new appointee will be appointed a mentor shortly after appointment.
You will be encouraged to be active in clinical research.
We are part of a network of hospitals that have been collaborating with Cranfield University to form the Cranfield Postgraduate Medical School (based at Redwood House in Gloucester).
Consultants at Gloucestershire Royal and Cheltenham General Hospitals currently hold professional honorary academic appointments.
You may be afforded the opportunity to join this multi-disciplinary postgraduate medical school as a faculty member depending on interests and research activity.
We have a well-established framework for clinical governance that incorporates the clinical audit program.
A wide variety of audits are undertaken within the Department.
The Department is supported by a dedicated audit assistant, a dedicated member of the Information department and a dedicated risk manager.
Clinical Governance meetings are regular.
The post-holder will be expected to take part in developing clinical audit activities within the department and achieving clinical governance objectives in accordance with the agreed Divisional and Trust clinical governance programs.
The Gloucestershire health community supports the development of the Gloucestershire Academy.
The Academy is hosted by the Gloucestershire Hospitals NHS Foundation Trust.
Initially led by the University of Bristol Medical School and now involving the University of the West of England Faculty of Health and Social Care, it will continue to grow to embrace the ambitions of the NHS, the University and other organisations that may be relevant to our countys Trusts and employees.
Meanwhile its implementation currently involves providing an academic infrastructure for students in medicine, nursing and the allied health professions to pursue the curriculum with relevant high education institution, in the health service, clinical setting of the secondary care Trusts of Gloucestershire.
Clinical Governance Our Trust has a well-established framework for clinical governance that incorporates our clinical audit program.
We are all expected to take part in developing clinical audit activities within the department to achieve clinical governance objectives in our agreed Divisional and Trust clinical governance programs.
to undertake all work in accordance with GHNHSFT policies and procedures to maintain CPD requirements for appraisal and revalidation to participate in our annual Job Planning review process to participate in annual appraisal including 5 yearly multi-source feedback to adhere to our Adverse Clinical Incident policy Leadership and management There is an expectation that consultants will rotate through directorate clinical management roles and this work will be part of Supporting Professional Activities (up 1 additional PA).
Help develop critical care services within the overall Trust Strategy.
Attend critical care delivery group and Clinical Governance meetings to develop cross county working.
To provide medical information for the development of systems appropriate for Trust needs.
To participate in departmental consultant and senior staff meetings whilst remaining within the framework of the agreed strategy of the Trust and the Department.
To attend other departmental, Divisional and Trust meetings as necessary.
To attend regional and national meetings as necessary.
To undertake all work in accordance with Trust procedures and operating policies.
Accountability The post holder will be immediately accountable to the Specialty Director.
The post holder will be expected to undertake appraisal and to respond to any identified development needs.
The post-holder has a general duty of care for their own health, safety and well-being and that of work colleagues, visitors and patients within the hospital.
This statutory duty is in addition to any specific risk management or clinical governance accountabilities associated with the post.
Finally, the post-holder is expected to: Observe the rules, policies, procedures and standards of Gloucestershire Hospitals NHS Foundation Trust together with all relevant statutory and professional obligations.
Observe and maintain strict confidentiality of personal information relating to patients and staff.
Be responsible, with management support, for their own personal development and to actively contribute to the development of colleagues.
Our Department of Critical Care (DCC) The DCCs of the two acute sites work as one service with shared aims, policies, procedures, equipment and budget.
The consultant staff work in both units either as the primary consultant or as a second consultant at Gloucester Royal covering HDU.
All consultants work as part of a 1:8 on call rota.
Medical, nursing and therapists all work across both units as part of the one service philosophy.
We are very proud of the service we deliver to our patients and the culture of excellence we promote.
Both units are rated outstanding by the CQC and consistently produce ICNARC SMRs between - The clinical lead is Dr Simon Webster The matron is Sr Debbie Elliott.
There is an outreach service (Acute Care Response Team) providing a comprehensive service to both hospitals, working closely with DCC.
We have a programme for the training and recruitment of Advanced Critical Care Practioners as part of the team (we have 4 qualified and 2 in training with a plan to build to 8 full time positions).
There is a growing research programme including participation in national multi-centre trials.
Cheltenham 12 beds including 4 side rooms.
Approximately 450 admissions per year.
Admits emergency and elective patients from medical and surgical specialties as well as regular admissions from the busy haematology/oncology unit.
Echocardiography (TTE & TOE) have been in established practice for 15 years and training courses are run including FICE.
Gloucester 19 beds including 4 side rooms and a dedicated Surgical HDU Approximately 1000 admissions per year.
Admits emergency and elective patients from medical and surgical specialties as well as a large and busy emergency dept.
Largest paediatric case mix in the south west region outside Bristol Childrens Hospital both retrieved and remaining on the unit in uncomplicated cases.
The Anaesthetic Department The department provides services to all general subspecialties across the two acute sites and several peripheral hospitals.
Many of the 69 consultants work in both acute hospitals.
Cheltenham Specialties: generalsurgery, colo-rectal, breast, elective orthopaedics, ophthalmology, major urology and gynae-oncology.
Acute & chronic pain Gloucester Specialties include: generalsurgery, colo-rectal, breast, upper GI (including oesophagectomy), vascular (including pateints from Swindon), spinal surgery, benign gynaecology, trauma, ENT, maxillo-facial, paediatrics, MRI, obstetrics Acute & chronic pain Person Specification Qualifications Essential Fellowship of the Faculty of Intensive Care Medicine or EDIC Fellowship of the Royal College of Anaesthetists MBBS or equivalent Full General Medical Council registration.
On Specialist Register for Anaesthesia via CCT or CESR-CP (Combined Programme) (proposed CCT/CESR-CP date must be within 6 months of interview) Desirable FICE/FUSIC/equivalent echo/US qualification MRCP ALS / ATLS EPALS or APLS Clinical experience Essential General training leading to CCT or equivalence/experience in Critical Care General training leading to CCT in Anaesthesia Desirable Experience in a range of critical care sub-specialities Experience in Acute pain Quality management Essential Ability to manage and balance risks Working knowledge of evidence-based practice Experience in audit and management of change Ability to deal with complaints from the public.
Experience in audit.
Desirable Experience of leading Quality Improvement projects Research Essential Ability to appraise research critically Ability to supervise junior staff undertaking research projects.
