This is an exciting time for the Trust as we are on a journey to bring together all NHS community and mental health services provided in Hampshire and the Isle of Wight, including services provided by Solent NHS Trust, Southern Health NHS Foundation Trust, Isle of Wight NHS Trust, and Sussex Partnership NHS Foundation Trust, in what we call project Fusion.
Main Duties of the JobThis is an ideal post for a geriatrician who enjoys being hands-on and working closely with colleagues in primary, secondary, and intermediate care.
There are opportunities to get involved in admission prevention, virtual wards, proactive community services for older people living with frailty, advance care planning, and end-of-life care.
The environment within the hospital is ideal for older patients who need high-quality medical and nursing care but not intensive medical intervention or referral to more specialist services.
A particular strength is end-of-life care.
