Job summary An exciting opportunity hasarisen within the Gloucestershire Continuing Healthcare (CHC) Team for a ComplexCase Manager (Band 7) registered professional.
The role is planned tobuild on a combination of Complex Case Management skills & subject matterexpertise, and to develop and support a growing service and work autonomously withina supportive team.In this role you will supportthe wider CHC team by case managing highly complex individuals and theirfamilies/carers helping them to access the most appropriate care to supporttheir needs.

The role includescase managing a variety of patients physical health, learning disability, mental health, ABI, etc.
in a variety ofsettings.You will work across the ICB footprint visiting patients ina variety of settings including their own homes as well as meetings on MSTeams.

Main duties of the job Day to day case management for CHCeligible patients with highly complex needs including monitoring high costpackages.Making decisions around complex CHCcases in accordance with the CHC National Framework.Court of Protection case managementwhich includes attendance in court representing the ICB.Working with care providers andsocial care in the best interest of the patient and to provide the best carepossible.Safeguarding highly complex andchallenging individuals.Working for the patient to achieveoutcomes that are best for them.

Registered Professional ( Nurse, OT, Physiotherapist, Social Worker) Post-graduate degree in Management Studies or equivalent.
Significant experience of successfully operating in a politically sensitive environment.

Experience of managing risks and reporting.
