Reporting to the Operations Manager, this is a varied role which involves direct communication with patients, relatives, ICBs and wider care teams.

Hold an NVQ Level 3 (or equivalent) in Health and Social Care or a related topic Have a proven track record in managing and coordinating care for clients in a community setting Experience in developing and implementing care plans Possess a supportive management style that can bring out the best in others Have advanced communication skills verbally and written Have a full UK driving licence for client visits The key responsibilities include...
