Complex Care Coordinator Location: Leeds City Centre Salary: £25K - £27K depending on qualification and experience Job Purpose: Leader Group is seeking an experienced and dedicated Complex Care Coordinator to oversee the effective delivery of complex care services to our clients in the community.
The role involves working closely with health care professionals, clients, and their families to develop and manage personalised care plans, ensuring the highest quality of care.
You will play a critical role in coordinating staff schedules, supervising the care team, and supporting both clients and their families.
Key Responsibilities: Care Coordination and Management: Coordinate and schedule care services, ensuring adequate staffing levels and continuity of care.
Collaborate with compliance and recruitment teams to maintain optimal staffing levels.
Arrange personalised rotas and visits for clients, taking their preferences and needs into account.
Liaise with health-care professionals, including doctors, nurses, and therapists, to ensure integrated care.
Monitor and assess the quality of care, identifying areas for improvement and implementing changes as needed.
Staff Supervision: Supervise and support care staff, ensuring they have the resources and training required for effective care delivery.
Conduct regular performance evaluations, providing constructive feedback and support.
Address performance issues promptly, fostering a positive and collaborative team environment.
Client and Family Support: Offer guidance and support to clients and their families, addressing any questions or concerns.
Educate clients and families on available services and resources to promote their overall well being.
Advocate for client's rights, ensuring their preferences are respected.
Team Collaboration: Foster effective communication and collaboration among the care team, including caregivers, nurses, and other health care professionals.
Participate in team meetings and case conferences to discuss client progress and optimise care strategies.
Documentation and Reporting: Maintain accurate and up-to-date client records, including care plans, progress notes, and relevant documentation.
Prepare reports on client outcomes, service utilisation, and other essential data, ensuring compliance with regulatory requirements.
Rota and Visit Management: Coordinate and manage staff rotas, ensuring appropriate coverage for client visits.
Assign staff to clients based on skills, availability, and individual client needs.
Regularly adjust rotas to accommodate client or staffing changes, and communicate updates promptly.
Training and Development: Identify training needs and coordinate training programs for care staff to enhance their skills.
Stay current with industry best practices and developments in complex care coordination.
On-Call Duty: Perform on-call duties as needed, responding to urgent or emergency situations outside regular hours.
Coordinate appropriate care and support for clients during on-call periods, maintaining effective communication with staff and escalating critical issues when necessary.
Qualifications and Skills: Proven experience in complex care coordination or a related role.
Strong organisational and communication skills, with the ability to multitask effectively.
Knowledge of healthcare regulations, care plans, and documentation.
Empathy, patience, and a commitment to providing high-quality care.
Level 3 in health and social care Valid UK full driving licence and access to a car.
