Job summary We are looking for anenthusiastic band 5 community staff nurse, to work with the OOH Districtnursing service 3 evenings per week, working to You will join a team of nursesdelivering high quality care to patients who are housebound.
Youwill be caring for patients with long term conditions and complex health needs,developing your skills and knowledge, whilst working towards an integratedhealthcare structure.
You should be able to demonstrate you can make clinicaldecisions to provide evidence-based practice and care planning in response toclinical assessments.
Main duties of the job The successful candidate will perform holisticassessments of patient nursing needs.
Plan, assess, implement and evaluate caredelivery according to changing health needs utilising prescribing skills.Ensure practice is evidence based throughundertaking audit and critically appraising literature to achieve a highstandard of care.The post holder actively supports the DistrictNursing team by undertaking responsibility for a delegated caseload.
Liaise with the District Nurse team leader toensure information sharing in relation to patient care / changes to care planand the continuity of care and reviewing of complex care packages.
Mentoring, Teaching and Assessing.Liaises with all health care professionals andother agencies to ensure effective service delivery.
