We have Community Staff Nurse positions available within the team in North Sedgemoor covering Highbridge, Burnham, Cheddar, Axbridge and all the villages in between.
Should you be successful the team cross-cover the whole area.The shift patterns are 8am-4pm, 9am-5pm 10am-6pm or 2pm-10pm.
You will also work one in four weekends which include additional unsocial hours enhanced payments.

Main duties of the job As a community nurse, you'll provide care to patients in their homes, managing a range of complex health issues and collaborating with various other disciplines.You'll also work closely with the Hospital @ Home team, focused on preventing acute admission.
You'll develop a broad range of nursing skills, including IV medication administration, palliative and end-of-life care, complex wound management, catheter and continence care, diabetes management, PICC line care, medication administration, and drain tube management.You'll need to make autonomous clinical decisions and create holistic care plans, with the support of your colleagues and Team Leaders.
Comprehensive training and guidance will be provided by our dedicated Clinical Lead nurses.

Job description Job responsibilities Community Nurses work as part of a team providing high quality and compassionate care to individuals.
The Community Nurse is an autonomous practitioner who is responsible for the holistic assessment of both complex and simple care needs, the development, implementation and evaluation of care and delegation of care where appropriate to other members of the team.
They will support the multidisciplinary team in the delivery of high-quality care to enable individuals to achieve their person-centred goals.The Community Nurse will take responsibility of the patients care pathway and non-clinical duties in the absence of the Team Leader.
They will have the understanding to address complex problems in their day-to-day work, reporting concerns to a Team Leader.The Community Nurse will be able to work effectively as part of a team and will always act within the limits of their competence, knowing who to ask for help and support as required.

Person Specification Qualifications Essential Relevant degree in Nursing or equivalent qualification Appropriately registered with the Professional body as per post requirements Evidence of good standard of literacy and English language skills Desirable Evidence of continuing professional development and to have undertaken post registration training, coaching and / or mentorship training Experience Essential Knowledge and experience of current national policies affecting the NHS Knowledge of audit and research skills Understanding of clinical governance Knowledge of physical & Mental health needs or know where to access appropriate advice Desirable Substantial experience in the required setting Ability to undertake the higher-level clinical skills required for the specialist area Demonstration experience in the required setting Demonstrate clinical and managerial leadership ability Experience of working in multi-agency services Experience of specialist working in District Nursing Additional Criteria Essential Able to demonstrate clinical reasoning skills to assimilate information to make a clinical judgement regarding diagnosis and intervention Ability to communicate in a professional way with people at all levels of understanding either face to face, on the telephone, e-mail, or written communication by dealing with incident reports or writing statements Compassionate - Open minded, treats colleagues, patients, carers and relatives with dignity and respect.
Dexterity and accuracy required for, , intravenous injections, syringe pumps and infusion, insertion of catheters and removal of sutures Recognising in yourself and others the impact that frequent exposure to distressing circumstances has on care and compassion and being able to respond professionally Ability to work autonomously Professional and patient focussed approach, acting as a role model to colleagues and junior staff members.
