Main duties of the job The post holder willbe responsible for the planning and delivery of patient care within AdultCommunity Nursing Teams.

To assess and plan individual patient care plans, education and treatment reviews.Contribute to the delivery of educational opportunities for health care support workers.
To act as a mentor for pre-registration nursing students.Demonstrate the appropriate supervision skills with mentorship, achievement of learning competencies and ongoing training and development.
Take responsibility for the delegated Appraisals and achievement of learning competencies for a designated number of support workers.To assist in supporting the coordination of community nursing services across Wakefield District, to ensure uninterrupted, seamless patient care, to support nursing colleagues in the provision of a high quality, 24 hour, 7 day per week nursing service.Clearly communicate timely, accurate and relevant information regarding patient care with patients, their families and carers, other professionals and services to ensure seamless, appropriate timely care and where appropriate empower patients to become partners and maintain their independence.Contribute to the development of strategies, guidance and protocols in accordance to MY clinical governance systems and objectives.
To communicate collaborative partnership working with services within the Trust, primary care and social services.
To take the lead in team developments as designated by their line manager.To perform risk assessments ensuring a safe environment within patients homes and work areas.
Ensures adherence to organisational Health and Safety, risk and governance arrangements.To maintain standards in ongoing health information, both electronic and paper based, ensuring safe keeping and confidentiality, accurate record keeping of relevant data and profiling.
To input clinical information onto System One as per MY policy.To maintain confidentiality, privacy and dignity in relation to the patients and their treatment at all times.To undertake all mandatory / essential training as required by MY.To be responsible for maintaining professional registration and continuing professional development.
Demonstrate a professional approach to work in accordance with the NMC Code of Conduct.To demonstrate and maintain own knowledge and awareness of recognising and responding appropriately to vulnerable adults.Review medications, seeking advice from local pharmacists and the drugs information hotline as appropriate.
If legally authorised, prescribe medications from agreed formularies based on efficacy, safety and cost.To participate in audit and research processes to ensure evidence- based health care is provided.Participate in clinical supervision ensuring that practice is maintained at high levels and is evaluated utilizing appropriate mechanisms.Contribute towards performance targets particularly in relation to reducing hospital admission and length of stay.Ensure equipment is obtained, used, and maintained in accordance with MY Medical Device Policy.To adhere to the lone working policy on order to minimise the risk associated with visiting patients that are unknown to the service.
To provide advice and support to health care support workers concerned regarding lone working.
Person Specification Qualifications Essential 1st Degree or willingness to work towards.
RGN Level 1.
Evidence of clinical update.
Relevant post registration qualification.
NMC Professional Registration.
Desirable Any further qualifications relevant to this post.
Willingness to undertake further relevant training.
Skills and Abilities Essential Ability to assess, plan, and implement nursing care packages.
Excellent communication skills to include verbal and written.
Ability to adhere to and follow guidelines and policy into practice.
Understanding/experience of interagency working.
