Main duties of the job Community nursing offers a different experience to traditional ward based nursing; it does provide the opportunity to deliver a wide range of nursing interventions to people in their own home and within residential care.
As a community nurse you will develop confidence and become expert in clinical skills including wound care and have the opportunity to deliver end of life care for people in their preferred place.The community nursing service is commissioned to provide a 24/7 service.
The core hours are between - hours, early and late shift patterns are also part of the working week.
Applicants will be required to participate in working other shifts as part of the role including weekend and bank holiday working on a rotational basis.We welcome applications from experienced staff wishing for a change in direction and from new registrants who we offer a supportive environment to complete a robust Preceptorship programme.The team describes working in the community:"You get to know your patients really well and give continuity of care because of caseload management".

Job description Job responsibilities Contribute to, support, and promote ECCHs, strategic direction, values, and culture in relation to community health services.
Discuss all treatment options with sensitivity, knowledge, and expertise.
To act as a patient advocate when appropriate, respecting patient confidentiality with privacy and respect for diverse cultural backgrounds and requirements.
Act as a resource for other health and social care professionals on matters relating to community nursing.
Participate in clinical supervision within Primary Care Home as required.
Participate and maintain a learning environment and maximise opportunities for education and development in the clinical area.
Develop excellent internal and external Network to promote Primary Care Home.
Contribute to the clinical governance agenda through participation in clinical risk assessment and clinical audit.
Contribute to an environment conducive to effective working, respecting, and supporting staff to deliver high quality clinical services.

Work as part of the multidisciplinary Primary Care Home to deliver planned and unplanned services.

Person Specification Qualifications Essential Professional Registration with Nursing and Midwifery Council (NMC) Registered Nurse Level 1 (Adult) BSc in Nursing Practice or equivalent Desirable Mentorship/ preceptorship qualification/ ENB 998 Evidence of working towards a nursing degree Experience Essential Evidence of continued professional development.
Ability to work unsupervised.
Ability to demonstrate clinical knowledge and competence within scope of clinical practice.
Previous experience of working in health care.
Desirable Recent clinical experience in community nursing.
