Job summary An exciting opportunity has arisen to become a Community Staff Nurse and join the Buckingham Adult Community Healthcare post is to provide nursing interventions to patients in their home environment predominantly supporting diabetic patients and out of hours patient calls.

You should have a wide range of clinical expertise within the NHS as well as experience working in a multidisciplinary team environment.
The post holder must be able to demonstrate their professional skills and competence in medicines management, care and treatment delivery.
You must be innovative in your approach and willing to work with other members of the community team and ward staff to ensure a high-quality service delivery.
You must be willing to develop your own knowledge and will need to have a full driving licence and access to a car with business insurance.
Main duties of the job o To contribute to processes for service and quality Assess the physical and psycho-social needs of patients and deliver therapeutic interventions based on best available evidence in order to improve health To manage own case load, delivering and evaluating service delivery to patients with a variety of medical conditions and those delegated by case To plan interventions with the Case Manager and other team members using the common assessment To take part in the referral management process to ensure patients are treated by the most appropriate member of the team/ To contribute to the development of protocols and procedures developed for clinical pathwayso Assist in the development of case management protocols and guidelines within their own area of professional practiceo Participate in weekend rotas / 7 day working rotaso Take part in the clinical rotation within the post holder's relevant profession.

Person Specification EDUCATION, QUALIFICATIONS & TRAINING Essential Registered Nurse (Level 1) Desirable Degree in Nursing Evidence of further continued professional development which may include courses: Interest in older people's nursing/ rehabilitation and awareness of the political agenda relating to advanced age Mentorship (ENB 998, SLICE, MSSP) EXPERIENCE Desirable Experience as a band 5 preferably in medical nursing within the NHS Previous experience as shift/team leader Experience of teaching, HCA's Students and qualified staff SKILLS, ABILITIES & KNOWLEDGE Essential Ability to communicate clearly with patients, relatives and the multidisciplinary team in English Ability to write clearly and legibly in English Ability to work required shift pattern Knowledge of up to date nursing and NHS issues Computer Literate Ability to work within a team and independently Managerial and leadership ability Good time management skills Desirable Competency in IV drug administration.
Infusion pump skills Knowledge of evidence informed and research based practice Ability to resolve conflict/dispute in a non -confrontational manner SPECIAL CIRCUMSTANCES Essential Ability to work internal rotation - Early, late, long day and night shifts including weekends
