The Hospital Team works in partnership with individuals to identify what matters to them.
This includes undertaking Mental Capacity Assessments, Continuing Health Care Assessments, leading Best Interest Meetings to ensure people have a safe and timely discharge from hospital.We work alongside a range of professionals within the teams, including colleagues from the health board, therapies and external providers.
