Job summary CDDFT Community Respiratory Nursing Service is pleased toadvertise this exciting opportunity for a wte Community Respiratory SpecialistNurse working across the South service including Durham Dales, Sedgefield andDarlington localities.
The successful postholder will deliver respiratory care torespiratory patients including COPD, Emphysema, Asthma, Bronchiectasis,and Interstitial Lung Disease, in out-patientclinics as well as in their own homes, supporting the community specialistrespiratory nursing service.The successfulcandidate will utilise their outstanding organisation and communication skillsto achieve best patient outcomes whilst being well supported by expertrespiratory sepcialists to develop a range of competencies in deliveringexcellent respiratory care and support for this group of complex patients.This post offers flexible working, Monday to Friday, 8:00amto 6pm and nominal base is Innovation House, Bishop Auckland.
Main duties of the job Undertake specialist, holistic, assessment, diagnosis,planning, implementation and evaluation of clinical care for a caseload ofpatients with complex respiratory disease.Beknowledgeable in respiratory disease, pathophysiology, function and management.Providespecialist advice to patients/carers.Be skilledin diagnostics including diagnostic spirometry and oxygen assessment, reviewand prescribing.Act as anaccountable practitioner.Maintainaccurate records as an autonomous practitioner.Act aspoint of contact for the wider local community teams.Provideeducation, mentorship and leadership for junior colleagues and students.Skilled atassessing and managing risk.Acceptsthe responsibility for decisions made and actions taken.Norequirement to manage/monitor budget.

Promoteservice development, improvements and quality expansion and efficiencies.Clinical Responsibilities To casemanage patients with a complex respiratory diagnosis within a community settingwithout direct supervision, reporting to the respiratory specialist nurse lead,as appropriate, utilising specialist history taking and examination skills.
Holisticallyassess patient need and develop a plan of care that is individualised and meetsthe needs of patients and carers through physical assessment skills and a respiratoryaccredited course.
Diagnosticspirometry performance and interpretation qualification and registration eitherthrough The Association for Respiratory Technology and Physiology (ARTP) or atcertified ARTP standard, is desirable but not essential as training will beprovided.
Possesssound knowledge of supplementary oxygen guidelines, assessment, review andprescribing through blood gas performance and interpretation.
Performall oxygen assessment and reviews as outlined through BTS guidance.
Identify,manage and review adjustments in optimising medication within agreed serviceprotocols, through non-medical prescribing.
Assistpatients to identify and manage risk factors using behavioural change toself-manage their condition.
Providespecialist advice to patients/carers in relation to programmes of care whichmay be complex.
Providesensitive information to patients, where there may be barriers to understand.
Decisionmaking/clinical judgment and problem solving.
Liase withmulti-disciplinary teams, referring as appropriate.

Specialist knowledge of respiratory disease including disease specific management pathways and self-management planning.
Accurately record clinical data and understand its significance: BP, heart rate and rhythm, pulse, oximetry, respiratory rate and peak flow rate.
Brief smoking cessation intervention.
Assess and train inhaler technique.
Knowledge of benefits of pulmonary Rehabilitation.
Knowledge of self-management and exacerbation management.
Phlebotomy or willing to work towards.
Ability to work independently and with minimal supervision.

Qualifications Essential Registered Nurse.

Specialist knowledge of respiratory disease including disease specific management pathways.
Physical assessment skills course or willingness to work towards within first 12 months of employment.
Independent prescribing course and prescribing experience or willingness to work towards within the first 24 months of employment.
Sound knowledge of national and regional policies influencing respiratory disease.
Accredited teaching qualification.
Desirable Cognitive Behavioural Therapy qualification or willingness to work towards.
