Community Reablement Assistant (Nights) - Myos House

We have an exciting opportunity to join the Home First Team based within Myos House, Extra Care Housing for People living with Dementia.
Our aim is to ensure that a person can remain at home, feel confident and is safe to live as independently as possible.
We work in partnership with other social care and health professionals to prevent care needs from escalating and works to prevent avoidable admissions to hospital and care homes.
Myos House 'helps people do things for themselves' rather than 'doing it for them' through support programmes that enables people to optimise their daily living skills and wellbeing.
The role will involve working with individuals and their circle of support to maintain their confidence and continue living well with Dementia - based on their abilities and strengths and those of their family and community.
We provide support 24/7, 365 ensuring the safety and wellbeing of vulnerable adults living within Myos House.
You will need to be solution focussed, be able to motivate others, have excellent decision making and interpersonal skills and be able to work autonomously in complex situations.
You will be part of a rolling rota, the shift time is 11pm-7am.
There is an expectation to attend regular mandatory training and meetings outside of your shift time as part of your role.
Post holders will be expected to be flexible in undertaking the duties and responsibilities attached to their post and may be asked to perform other duties, which reasonably correspond to the general character of the post and are commensurate with its level of responsibility including direct personal care.
This is temporary role, currently until 31st March 2025.
