Job summary Joining our Reablement Team as a Community Reablement Assistant Level2, youll help to support adultsover the age of 18 across Hampshire who may be experiencing a need foradditional care and support.
Our aim isto maximise their independence and potential, as well as reduce, avoid or delaythe need for ongoing care whilst supporting them to remain living at home.
Our team of Community ReablementAssistants, Occupational Therapists and Sensory Officers, work inpartnership with Health colleagues to offer support, encouragement and guidanceto those with short or long term conditions andfrailties as well as supporting those coming out of hospital.
Main duties of the job What youll do:Drive to service users' homes and working independently carry out reablement intervention.Work towards enabling service users independence in daily living tasks and personal careDeliver hands on careWrite up medicationUndertake risk assessmentsComplete and review service users goalsComplete working supervisions with Community Reablement Assistants Level 1s and support their developmentUse a variety of IT systems such as MS Teams and email.What were looking for:Experience of working within a social or health care settingEffective communication skills both verbal and writtenSound literacy and numerical skillsSound IT skillsMotivated and work on own initiativeGood observation, listening and feedback skills with the ability to seek advice from other appropriate professionalsGood team playerAble to prioritise delegated workload and demonstrate effective time management skillsLeadership skillsWillingness to be inducted and trained in enabling skillsIn return, youll receive professional ongoing training, a comprehensive induction and the full support of the wider team.
About us Working PatternsOur Service runs 07:00-22:00, 7 days a week, 365 days of the year, therefore youll be required to work evenings and/or public holidays and/or weekends as part of your Normal Working Hours for which enhancements are paid in line with the HCC salary policy.
