Job summary To support the delivery of the Healthy Child Programme (HCP) 0- 19 (DH, 2010) thereby improving outcome for children and young people and to the Public Health agenda, meet Public Health targets and reduce health inequalities in partnership with key agencies meeting National agendas Promote the protection of children, adhering to the Local Safeguarding Children's Partnership policies and procedures.
Working under the supervision of a band 6 Health Visitor/School Nurse within a designated area, alongside partner agencies such as Action for Children, The Children's Society, Family Hubs, Children's Services, and education systems.
Work as part of a team to ensure that Key Performance Indicator (KPI's) targets for the service are met, to support service development, standard setting, clinical audit and bench marking, deliver a high-quality need led, safe and effective service to all client groups.
Main duties of the job Participate in the assessment of health needs and formulation of care plans for children, young people, their families and their communities.
The includes the implementation of programmes of care, offering support and advice to parents and families in the home, clinic school or community setting.
In collaboration with the public health nursing team, identify individuals and groups within the community where collaborative working and advice would forward the interests of public health and promote healthy living, in particular seeking to identify disadvantaged groups and address health inequality.
Offers advice and support to children, young people and adults in accordance with evidence-based practice.
To actively take responsibility for appropriately identifying, accessing, and participating in supervision, according to individual need ie.
Child protection, CAMHS, managerial.
Support the delivery of KPI's by working collectively across the area.
Under the direction of the Specialist Community Public Health Nurse (SCPHN) complete Early Help assessments, and work with the multi-agency team to support those children, young people and their families in need of additional support and those at risk of significant harm, including participating in Team Around the Family meetings, and supporting Child Protection Meetings (where appropriate).
This includes undertaking the role of lead professional or key worker for children and their families with the most complex needs.
About us The 0 to 19 Torbay Public Health Nursing team works in collaboration with Action for Children and the Children's Society to provide preventative and supportive healthcare to children, young people and their families within Torbay.
This includes the monitoring and provision of targeted support to vulnerable children and those whose welfare and safety may be at risk.
The post holder works as a member of the 0 to 19 Torbay Public Health Nursing team maintaining a public health focus under the supervision of a Specialist Community Public Health Nurse to deliver the Healthy Child Programme in a variety of settings including the home and community settings.
