Specialist teams work closely with patients, their carers and families to put together a plan of care which suits each individual person and offers different types of support including community, inpatient, outpatient and day services.As a Community Psychiatric Nurse with NHS professionals, youll be working a caseload of patients with one of our community services Main duties of the job What youll be responsible for:Will act as an independent practitioner within a defined community setting, while liaising with other professionals as necessaryWill manage a defined caseload and provide supervision to others as appropriate.You will be involved in providing a high level of safe and effective careBe responsible for the assessment, planning, implementation and evaluation of careAdhere to all Trust policies, procedures and guidelinesLiaise with other community service and multidisciplinary teamsProvide mentorship and support to others.The role will involve, but not be limited to you performing the above.

Person Specification Qualifications Essential Mental Health Nursing Degree or Diploma Experience Essential NMC Registration 6+ Months Community Psychiatric Nursing experience within UK (NHS or Private Mental Health Provider) within the last 3 years.
