Our interventions areexciting and dynamic, supporting patient flow through rapid response/admissionprevention work and facilitation of hospital discharges with a Home Firstapproach.
With training you will gain furtherexperience in falls prevention, palliative and end of life care, and long-termcondition management including: Neuro, MSK, Orthopaedics, Respiratory,Rheumatology and Frailty.
Main duties of the job The service functions over 7 days from8am and 6pm.
We promote a positive work/life balance and utilise flexibleworking to accommodate needs, your role will require lone working.
We haveposts in the North and West of the city dependant on preference and serviceneeds.
You will be responsible for your owncaseload, delivering holistic rehabilitation made meaningful by taking place inlocal communities, delegating to others to optimise outcomes.
You will haveopportunity to make a positive difference to your patients lives and developskills in triage, rapid response, management of deteriorating patients andlong-term conditions.
You will work closely with the wider multidisciplinaryteam to enhance your knowledge, supported by clinical supervision, peer supportand regular CPD.
You will develop leadership skills supporting others includingtherapy assistants and students and contribute to service improvement projectsacross the Trust and wider partners.
