Job summary Cheshire Wirral Partnership NHS Trust are looking to appoint an palliative clinical nurse specialist to work within the Community Specialist Palliative Care Team within West Cheshire .
The team consists of Clinical Nurse specialists ,Occupational Therapy, Physiotherapists, a Therapy Assistant and administration staff.
Experience of working in the community is desirable as the role is based supporting patients in their own homes.
We are looking for a well-motivated Clinical nurse specialist with excellent communication, organisation and leadership skills.
The successful candidate will receive a full supportive induction when they join the team and will work closely with the wider MDT.

**We offer a relocation package of up to £8,000** The relocation package is for anyone who would need to relocate in order to take up employment with Cheshire and Wirral Partnership.

Main duties of the job The post holder will provide holistic care and support to patients with complex palliative care needs with malignant or non-malignant life limiting conditions, working autonomously and as part of a team.
They will deliver care to patients registered with a West Cheshire GP in their own home.
The post holder will provide advice, education and support to patients, their carers and other health care professionals.
They will also promote advanced care planning, management of complex end of life symptoms and patient and carer expectations whilst meeting the holistic care needs of the patient.
Over recent years there has been a lot development within the service and the successful candidate will need to share the teams drive to take the service forward.
You also need to be passionate about delivering quality care to people receiving palliative or end of life care.
As the role involves the assessment and treatment of patients within their own home access to transport for work purposes is essential.
This a permanent full time post working for Cheshire Wirral partnership which is based at the hospice of The Good Shepherd, Chester.

Person Specification Qualifications Essential Registered General Nurse First level degree in related subject or undertaking a Degree Pathway Post-graduate qualification in palliative care or relevant subject or working towards Evidence of continued professional development Desirable Research qualification Advanced communications skills training or prepared to work towards Recognised teaching qualification or prepared to work towards Evidence of Masters level learning or prepared to work towards Knowledge and Expertise Essential High level of clinical and technical knowledge relating to palliative care.
Teaching/assessment and presentation skills Exemplary written communication skills Experience Essential Relevant post registration experience, at Band 5 or above in palliative care.
Experience of multi-professional working Experience of teaching Evidence of good communications skills Experience of supporting and influencing change.
Desirable Experience of audit and research Understanding of Macmillan Cancer Support and its role across the UK Leadership and management experience
