Job summary District Community Nursing Sister/Charge Nurse - Band 6 Salary Range: £33,338 to £47,084 Location: Reading, Newbury, Wokingham, Bracknell, Slough, Ascot, Windsor & Maidenhead.

Ideally the candidate will bring with them clinical experience of working with adults in a community setting.
In return we will offer an excellent package including the opportunity to further train and develop, and will support you with studying for and obtaining an Specialist Practice Qualification (SPQ).
Your working hours will be Monday to Friday (with some weekend and late shifts work on a rotation basis).

You will need to either have access to a vehicle which can be used for work purposes or are you able to travel as required to meet the needs of the service.

Main duties of the job We are seeking an enthusiastic and experienced nurse, with a warm personality and great people skills.
You will be willing to undertake relevant training, have leadership experience and be able to demonstrate innovative practice.
Show effective management skills, you will take responsibility for caseload management of housebound patients with multiple nursing need in the community.You will be responsible for ensuring high quality evidence-based care for patients, their families, and carers in the community.
Responsible for a complex and diverse caseload, actively working to prevent admission to the acute sector and supporting patients in their preferred place of care.Responsible for maintaining detailed, accurate and confidential patient records, adhering to Trust policies.Leading an effective supportive team, working together to provide clinical advice and support to other staff and patients.Responsible for the management of a defined caseload of patients, with continuing responsibility for the assessment of care needs, the development, implementation, and evaluation of care programmers.Accountable for the standards of care and for determining the activity of your team.
This will include acting as a role model for staff, sharing learning and developing practice.

Person Specification Education/Qualifications/Training Essential Registered Nurse - Adult Active on the Mentor Register Evidence of Leadership & Management development Recognised coaching qualification or willing to undertake Desirable Specialist Practitioner qualification (DN Degree, diploma or Certificate in District Nursing) Non-Medical Prescriber/Nurse Prescriber Qualification Continuous Professional Development Essential Evidence of continuing professional development (CPD) Evidence of a current CPD portfolio Previous Experience Essential Experience in mentoring and assessing both pre and post reg.

Evidence of clinical leadership within a community nursing environment including Mentoring/assessing in practice Recruitment and selection Risk and Incident Reporting (Root Cause Analysis) and supporting teams in learning from findings.
