Job summary We are recruiting for a Community Nurse Team Leader to join our team in Chipping Norton where you will have accountability and responsibility for the district nursing team.You'll be key in providing strong clinical leadership, ensuring the delivery of expert evidence-based nursing care to patients whose needs are best met in the community, their own home or other care setting.If you're an experienced registered nurse with a strong background in district nursing and clinical leadership, we would love to hear from you!A minimum of 30hrs per week may be considered.Cross locality working and flexibility is required for this role.Applicants must have a full valid driving licence and have access to a vehicle for work.

Main duties of the job Provide strong clinical leadership and management of the team Accountable for the management, development, and supervision of identified staff within the community team Overall accountability and responsibility for the named district nursing team Clinical holistic assessment and treatment of complex patients Work in partnership to reduce hospital admission and facilitate discharge Promote optimal levels of independence and wellbeing for patients.
