Do you want to make a difference in patient care?We are looking for a Band 5 Community Nurse to work within Countywide Out of Hours community Nursing team .In these roles, you would be working at part of the wider Community Neighbourhood team which includes District/Community Nurses, Physiotherapists, Occupational Therapists, Mental Health Nurses and Team coordinators.
Linking closely with colleagues including general practitioners and other members of the multidisciplinary team to support excellent nursing care at home, reducing the necessity for acute hospital admission.

Job description Job responsibilities Please note for this role you will be required to travel independently around the county meeting strict time deadlines.
You will need to hold a full UK driving licence* and have use of a vehicle.
You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs.

Ability to safeguard and promote welfare of children and young people/ vulnerable adults.
Good communication skills Appreciates the significance of safeguarding and interprets this accurately for all children and young people/ vulnerable adults whatever their life circumstances.
Desirable Understanding of clinical supervision Knowledge & Understanding Essential Has a realistic knowledge of personal strengths and areas for development Demonstrates empathy for the concerns of others Accepts responsibility and accountability for own work and can define the responsibilities of others Responsible for maintaining registration with NMC and/or other professional bodies Physical Requirements Essential Kneel, bend & stoop, and work in cramped environments Travel around work base locality Move and manoeuvre people and equipment Manoeuvre patients using safe techniques in accordance with care plan.
