Working hours are from 07:45-22:00.Working as a member of the team, you will provide holistic, high quality, clinically effective care to patients, working under the guidance and support of the Team Leader and Team you are a Registered Nurse or you are thinking about returning to practice after a career break or following retirement, this is the perfect opportunity for you to engage directly with our nursing teams across our our staff are encouraged and supported to develop within their roles, to meet the changing needs of the service and their own personal and professional development goals.
Main duties of the job Community Nurses are highly skilled practitioners who carry out interventions which include:-o Holistic assessment of needso Wound careo End of life and palliative careo Lower limb assessment and managemento Diabetes managemento Bladder and bowel careo Safeguarding/ MDT workingo IV therapyo Admission avoidanceo Discharge facilitationo Sepsis screeningThe successful candidate must demonstrate excellent clinical and communication skills to provide a high standard of care to ensure a positive patient experience.Experience in community care is an advantage however the post is also suitable for newly qualified nurses.
We have an excellent preceptorship programme which all our newly qualified staff will follow.

Person Specification Qualifications Essential First level registered nurse/newly qualified nurses would be considered for the post Diploma in health related area or equivalent experience Completion of Band 5 preceptorship programme (evidence may be required from RNs who have been registered for less than 2 years or with gaps) Knowledge Essential Understanding of NMC standards and implications for practice Experience Essential Multi-disciplinary/multi-agency working Skills Essential Excellent communication both written and verbal Evidence of effective interpersonal skills Ability to work independently in community setting
