Job overview We are looking for a Band 6 nurse to join one of our Community Nursing teams based in ABC PCN (Maidstone), to support the development of this service.
The service operates seven days a week from 08:00 - 16:00, with occasional late shifts of 10:00 – 18:00 on a rotational basis.
Weekend working is generally on a 1:4 ratio with days off during the week.The team support patients registered with GP practices working in the Maidstone town and outlying villages within the ABC Primary Care Network.Each team has two senior nurses to take responsibility for a defined geographical area, and provide clinical leadership to their Band 5, Band 4 and Band 3 staff.
The team will work together to manage caseload patients delivering a high-quality service.Staff will be required to drive their own car and are able to claim mileage costs and access the NHS car lease provision, following their probationary period.
Main duties of the job Using the named nurse model, we want to allow nurses to spend more time with patients, promoting self-management where possible, and improving patient lives.

Person specification Qualifications Essential criteria Registered health care professional (NMC registration).
Health related degree and evidence of relevant post graduate development at Level 6.
Evidence of Specialist qualifications relevant to area of practice (examples specialist practitioner district nursing, non-medical prescribing; mentorship, coaching, clinical expertise).
Desirable criteria PTLLS/DTLLS, PGCE or Certificate in Education.
Management or supervisory qualification or experience of study.
Working towards Master level study.
Experience Essential criteria Minimum of 5 years' experience in area of practice.
Practical experience in managing health related projects/complex caseloads.
Supporting others (mentor / educator / supervisor) experience.
Desirable criteria Experience of leading and managing teams.
Experience with the use of Microsoft Office and a variety of IT packages (E-Pay / E-roster/ AT-P/ RIO).
Knowledge Essential criteria Understanding of National Service Frameworks and NICE guidance to relevant to care setting and its application across health care services.
Evidence of continued relevant professional development and implementation in practice within the last 12 months.
