Main duties of the job Working within the Community nursing teams, you will undertake duties involving delivery of nursing services and holistic assessments of patient's health needs using a person-centered approach including providing quality and seamless nursing care to housebound patients and being proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.In addition, you will have the opportunity to be involved in facilitating self-care for patients with long term conditions, health promotion, and complex out of hospital patient care.
Desired SkillsAll Community nursing interventions for housebound patients, predominantly:Holistic Nursing assessment and ongoing care planning liaising with the team and attendance at safety huddle.Medicines administration including DiabetesManagement (insulin), palliative care anticipatory medicines and syringe driver administration.Frailty and OPAT Team CompetenciesTo provide holistic nursing assessmentsTo follow the escalation plan for the deteriorating patientTo provide patient centred careAssume delegated responsibility for holistic assessment and the planning, implementation, monitoring and evaluation of programmes of care/ care packages, contributing to complex assessment/ care packages.To help the patient towards self-care and maximising independence, maintain their optimum level of health and prevent further deterioration.
About us Essential for the role are- Clinical experience of IV therapy delivery, management of vascular access devices, experience in a community setting, venepuncture and cannulation skills and IV administration skills.We can offer bank shifts for you to book and choose at your convenience, training and support in practice to meet the requirements for registration and to meet personal needs.
